Missed Surveillance CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 Reactor Power 85%

DESCRIPTION OF OCCURRENCE:

During a records review, it was determined that required surveillance on the fire pump diesel 24V starting battery was not performed or documented.

The 92 day requirement of Surveillance 4.7.10.l.3b requires the specific gravity be verified appropriate for continued service of the battery.

In addition, the 18 month requirements of Surveillance 4.7.10.l.3c were not completed which includes a visual inspection for physical damage or abnormal deterioration of the cell plates and battery racks and to ensure the battery-to-battery and terminal connections are clean, tight, free of corrosion and coated with anti-corrosion material.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Personnel Error ANALYSIS OF OCCURRENCE:

When sent to perform the 92 day surveillance, the electrician did not completely understand the requirement of the surveillance and only performed a specific gravity test on the prolet cells, a routine weekly inspection.

In addition, after inspecting the battery, as required every 18 months, the electrician did not document his inspection because he routinely performs an inspection weekly and had not been required to document the inspection previously.

CORRECTIVE ACTION

The required surveillances were completed and documented immediately upon identifying the need.

The electrician was counseled on the importance of understanding his assignment prior to performing the surveillance.
