12 Fan Coil Unit Failure CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 Reactor Power 100%

DESCRIPTION OF OCCURRENCE:

Upon starting 12 containment Fan Coil Unit (FCU), it was observed there was no service water flow indication.

At 2235 hours0.0259 days <br />0.621 hours <br />0.0037 weeks <br />8.504175e-4 months <br />, the FCU was declared inoperable and Action Statement 3.6.2.3 was entered.

It is required to restore the inoperable FCU to operable status within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> or be in hot shutdown within the next 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Failure of Solenoid Valve 624 Coil ANALYSIS OF OCCURRENCE:

It was determined the solenoid valve 624 for 12SW223, the flow control valve had failed.

CORRECTIVE ACTION

The valve was replaced in kind from stock, tested satisfactorily and the Action Statement was terminated at 0500 hours0.00579 days <br />0.139 hours <br />8.267196e-4 weeks <br />1.9025e-4 months <br /> on July 3, 1980.

A Design Change, lEC-717, has been identified which will result replacement of this valve design with a more reliable solenoid.
