Boric Acid Storage Tank Level Below Minimum Required CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 Reactor Power 50%

DESCRIPTION OF OCCURRENCE:

In the process of transferring a concentrated boric acid solution to the Boric Acid Storage Tanks (BAST) using Procedure II-3.3.5, Boric Acid Solution Preparation and Transfer, the level in the operable BAST was allowed to be below the minimum level required by Technical Specification Action Statement 3.l.2.8(al), 5,106 gallons.

With a Boric Acid Storage System inoperable, it must be restored to operable status within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> or be in at least Hot Standby within the next six hours and borated to a shutdown margin equivalent to at least 1%

~K/K at 200°F.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The procedure used failed to caution the operator to ensure a sufficient volume of water was available to make the transfer.

ANALYSIS OF OCCURRENCE:

The procedure used, II-3.3.5, requires when a batch of concentrated solution is transferred, one BAST is removed from service to receive the batch of solution.

When the batch is transferred, it is then recirculated and sampled to ensure the BAST system will not be out of specification due to boron concentration.

Past practice had the operators raise the level in the BAST to remain operable above the minimum Technical Specification leve~ prior to removing the second BAST from service.

This was not done on this occasion and the BAST in service was left at 51% full instead of the required 70%.

CORRECTIVE ACTION

The transfer of solution was completed and the BAST was returned to operable status at 2000 hours0.0231 days <br />0.556 hours <br />0.00331 weeks <br />7.61e-4 months <br /> on June 12, 1980.

A review of the occurrence identified the violation of Technical Specifications and a procedural change was made requiring the level in the BAST remaining operable to be increased above the minimum level required prior to transferring a batch of boric acid solution.
