  1. 1 Fire Pump Inoperable CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 Reactor Power 100%

DESCRIPTION OF OCCURRENCE:

At 1214 hours0.0141 days <br />0.337 hours <br />0.00201 weeks <br />4.61927e-4 months <br />, the #1 Fire Pump was removed from service due to a loud noise coming from the diesel.

Action Statement 3.7.10.l(a) was entered at that time which requires a special report to be submitted within 30 days outlining the plans and pr0cedures to be used to provide for the loss of redundancy in this system if the pump is inoperable for longer than seven days.

The pump was returned to service on June 19, 1980, at 1940 hours0.0225 days <br />0.539 hours <br />0.00321 weeks <br />7.3817e-4 months <br />; 9 days, 7 hours8.101852e-5 days <br />0.00194 hours <br />1.157407e-5 weeks <br />2.6635e-6 months <br /> and 26 minutes after removing it from service.

DESIGNATION OF APPARENT. CA.USE OF OCCURRENCE:

The unidentified noise was identified during a subsequent inspection to be caused by a failure in the front bearing on the balancing weight drive shaft.

ANALYSIS OF OCCURRENCE:

A subsequent disassembly and inspection with the vendor representative present revealed the front bearing on the balancing weight drive shaft had failed along with the front balancing weight.

It was determined that a backup was necessary since the repairs would take longer than seven days.

A temporary pump was provi.ded and positioned for use if necessary.

It was not used.

CORRECTIVE ACTION

The Fire Pump Diesel was disassembled and inspected.

It was determined the balance shaft, bearings and counter weights needed to be replaced.

The necessary parts were installed.

The pump was tested satisfactorily and returned to service.

As a precaution, #2 Fire Pump Diesel was also* inspected.

Some wearing was noted on the front bearing of the balance weight drive shaft and was, therefore, replaced as a precaution.

The #2 Fire Pump was tested satisfactorily and returned to service.
