Failure of Steam Generator Wide Range Level Transmitter CONDITIONS PRIOR TO OCCURRENCE:

.Mode 1 Reactor Power 100%

DESCRIPTION OF OCCURRENCE:

At 1145 hours0.0133 days <br />0.318 hours <br />0.00189 weeks <br />4.356725e-4 months <br />, #13 Steam Generator wide range level transmitter failed resulting in the indication failing high.

Action Statement 3.3.3.5 was entered which requires the inoperable channel to be restored within 30 days or be in Hot Shutdown within the next 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The level transmitter failed.

ANALYSIS OF OCCURRENCE:

An investigation revealAd a transistor within the D/P transmitter failed causing the false indication.

CORRECTIVE ACTION

The transmitter*was replaced in kind from stock, tested satisfactorily and the Action Statement terminated at 1315 hours0.0152 days <br />0.365 hours <br />0.00217 weeks <br />5.003575e-4 months <br /> on July 3, 1980.
