Failure to Perform a Quadrant Power Tilt Ratio CONDITIONS PRIOR TO OCCURRENCE:
Mode 3 DESCRIPTION OF OCCURRENCE:
In preparation to change from Mode 3 to Mode 2, it was determined a Quadrant Power Tilt Ratio had not been completed within the required seven days.
Power was greater than 50% within the seven
.day period just prior to the planned mode change.
DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:
Procedural Inadequacy ANALYSIS OF OCCURRENCE:
The Operations Directive Manual is used as a tickler system to remind the operators of surveillance requirements more frequent than once per seven days, but did not provide a check list to ensure their completion.
The Quadrant Power Tilt Ratio was completed on May 26, 1980 and was not completed again prior to a unit trip on June 8, 1980 due to an oversight of the need to complete the surveillance requirement.
CORRECTIVE ACTION
The Surveillance Requirement was identified as an item necessary to complete as soon as the necessary conditions were attained.
The surveillance was completed at 1937 hours0.0224 days <br />0.538 hours <br />0.0032 weeks <br />7.370285e-4 months <br /> on June 11, 1980.
A check list has been provided of all Operations Directive Manual routine items which requires each item to be performed when appli-cable and initialed indicating review and completion.
This check list becomes a part of Operations Log #1 and is reviewed by the shift licensed operators at shift relief.
