Failure of Meteorological Instruments CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 Reactor Power 100%

DESCRIPTION OF OCCURRENCE:

Company 08038 During routin~ operations, it was determined that the 150'-33' Air Temperature ~T* had failed and Action Statement 3.3.3.4(a) was entered at 0040 hours4.62963e-4 days <br />0.0111 hours <br />6.613757e-5 weeks <br />1.522e-5 months <br />.

With one or more required meteorological monitoring channels inoperable for more than seven days, prepared and submit a special report to the Commission.

At 1605 hours0.0186 days <br />0.446 hours <br />0.00265 weeks <br />6.107025e-4 months <br />, the Action Statement was terminated since no problem could be found and the instrument operated properly.

On June 6, 1980, the same instrument failed a second time and it was determined the sensor had failed and the Action Statement entered at 0720 hours0.00833 days <br />0.2 hours <br />0.00119 weeks <br />2.7396e-4 months <br />.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment Failure.

The ~T sensor failed after extended use.

ANALYSIS OF OCCURRENCE:

Apparently the sensor failed oh June 4, 1980 and then gave indications of proper operation so that no cause for the instrument failure could be determined.

When troubleshooting the failure on June 6, 1980, it was determined the sensor had failed and needed replacement.

CORRECTIVE ACTION

On June 9, 1980, the service representative replaced all meteorological instrument sensors and they were tested satisfactorily.

The Action Statement was terminated on June 9, 1980 at approximately 1500 hours0.0174 days <br />0.417 hours <br />0.00248 weeks <br />5.7075e-4 months <br />.

An inspection order has been submitted to have all the sensors replaced on an annual basis in the future.
