No. 12 Containment Fan Coil Unit Inoperable CONDITIONS PRIOR TO OCCURRENCE:

Unit 1 was operating at 100% reactor power with an electrical load of 1140 MWe gross.

DESCRIPTION OF OCCURRENCE~

No. 12 Fan Coil Unit was in service when service water flow through the unit decreased to zero.

Technical Specification Action Statement 3.6.2.3 was entered at 0450 hours0.00521 days <br />0.125 hours <br />7.440476e-4 weeks <br />1.71225e-4 months <br />.

The Action Statement says three

  • independent groups of containment cooling fans shall be operable with two fan systems to each of two groups and one fan system to the third group.

With one group of containment cooling fans inoperable, restore the inoperable fan group to operable status within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> or be in hot shutdown within the next 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment failure of solenoid valve SV-624.

ANALYSIS OF OCCURRENCE:

Technical Specifications allow operation in a degraded mode for 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />.

No. 12 Fan Coil Unit was inoperable for about 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />.

The Action Statement was terminated at 1645 hours0.019 days <br />0.457 hours <br />0.00272 weeks <br />6.259225e-4 months <br /> on April 25, 1980.

CORRECTIVE ACTION

The solenoid valve was replaced with one from stock.

Surveillance Procedure 4.6.2.3 was performed to prove operability and the Action Statement was terminated at 1645 hours0.019 days <br />0.457 hours <br />0.00272 weeks <br />6.259225e-4 months <br />.
