--After entering Mode 4 at the completion of a refueling outage, a verification of the valve lineup to the Service Water System was not completed for the months of January and February 1980.
A verification was performed in December, 1979, just prior to entering Mode 4 and when identified, a satisfactory valve lineup was completed.

Missed Surveillance, Service Water Valve Lineup CONDITIONS PRIOR TO OCCURRENCE:
Mode 1 Reactor Power 100%
Unit Load 1140 MW DESCRIPTION OF OCCURRENCE:
The Technical Specification 4.7.4.l(a) requires verifying, when in Modes 1 through 4, two service water loops operable every 31 days.
Each valve servicing safety related equipment that ~s not locked, sealed, or otherwise secured in position, must be in the correct position.
The surveillance was not performed in January or February of 1980 but was completed in March and subsequent months.
DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:
Personnel error.
A timely review of items identified as outstanding was not completed.
ANALYSIS OF OCCURRENCE:
Due to an extended period of time in Mode 5, the computer updates were not submitted and the last data indicated completion in July 1979.
A review process was not re-established at the completion of the refueling outage and the surveillance was not identified as being overdue.
During an independent station audit, the item was identified.
CORRECTIVE ACTION
When identified, the surveillance was completed and documented.
A copy of the outstanding items list, a listing of any inspection orders outstanding at the time of the report, will be sent to the Senior Reactor Operators on shift for their review.
