Auxiliary Feedwater Valve, 14AF10, Out of Position CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 Reactor Power 100%

Unit Load 1140 MW DESCRIPTION OF OCCURRENCE:

At 1308 hours0.0151 days <br />0.363 hours <br />0.00216 weeks <br />4.97694e-4 months <br />, the Resident NRC Inspector discovered 14AF10, the manual isolation discharge valve of 13 Auxiliary Feed Pump to 14 Steam Generator, closed.

The valve position was verified and locked open, its normal position, immediately upon notification to the Senior Shift Supervisor.

This valve, 14AF10, is required to be open by Technical Specification 3.7.l.2(b) which requires the steam driven pump and the associated flow path to be operable or be in Hot Shutdown within the next 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />.

A complete system flow path verification was performed by an unlicensed operator, with a second independent verification performed by the Shift Technical Advisor, all valves were found to be in their required positions.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Personnel error.

An operator closed 14AF10 and started to close 13AF10 for a troubleshooting investigation of the steam driven Auxiliary Feed Pump governor control when he was informed to keep those valves open.

In his haste to continue his work assignments, he apparently failed to reposition the 14AF10 valve to locked open.

When a second operator performed a system valve lineup, he apparently recognized 14AF10 was locked and assumed it was open when it was indeed locked closed.

ANALYSIS OF OCCURRENCE:

On April 10, 1980, at 1627 hours0.0188 days <br />0.452 hours <br />0.00269 weeks <br />6.190735e-4 months <br />, ll-14AF10 and ll-14AF11 valves were closed in preparation for a 48 hour5.555556e-4 days <br />0.0133 hours <br />7.936508e-5 weeks <br />1.8264e-5 months <br /> endurance test of 13 Auxiliary Feed Pump.

At 1735 hours0.0201 days <br />0.482 hours <br />0.00287 weeks <br />6.601675e-4 months <br />, the endurance test was aborted 15 minutes after its start due to AFP governor control hunting.

All valves were positioned to their normal position at approximately 1855 hours0.0215 days <br />0.515 hours <br />0.00307 weeks <br />7.058275e-4 months <br /> while Performance Department personnel investigated the governor control

problems.

At 1915 hours0.0222 days <br />0.532 hours <br />0.00317 weeks <br />7.286575e-4 months <br />, a test run was requested by Instrument and Controls.

To perform the test run, an operator was assigned to manually start the pump.

He started to close ll-14AF10's.

He was informed, after closing 14AF10 and starting to close 13AF10, by the Nuclear Control Operator, that the AFlO valves were to remain open and ll-14AF11 control valves were to be shut from the control room.

13AF10 was opened, but in haste 14AF10 apparently was left locked closed, an abnormal position.

At the completion of the test, a different operator was directed to realign the sys-tem and perform a valve lineup verification of the system.

Ap-parently, when checking 14AF10 and seeing it locked, he assumed it was locked open and noted that on the valve lineup.

The valve line-up was completed, and 13 Auxiliary Feed Pump was returned to oper-ability at 2150 hours0.0249 days <br />0.597 hours <br />0.00355 weeks <br />8.18075e-4 months <br />.

The system remained untouched until 1308 hours0.0151 days <br />0.363 hours <br />0.00216 weeks <br />4.97694e-4 months <br /> the following day when the Resident NRC Inspector reported his find-ings.

CORRECTIVE ACTION

The valve was positioned properly and two independent valve lineups were completed for the system valves to ensure its operability.

A task force was commissioned by the Public Service Electric and Gas Company to perform an independent investigation of the occurrence and to submit positive recommendations.

Further action will be taken upon completion of the evaluation now being conducted.

Our response to the NRC inspection addressing this item will serve as a supple-mental report and provide definitive actions that will be implemented.
