Failure of vital heat tracing.

CONDITIONS PRIOR TO OCCURRENCE:

Mode III Reactor Power 0%

Unit Load 0 MW DESCRIPTION OF OCCURRENCE:

During a routine surveillance of the vital heat tracing in accordance with surveillance requirement 4.5.4.2 it was determined circuit 603 was not operating properly.

Temperature was 142°F instead of the re-quired 145°F minimum.

Action statement 3.5.4.2 was entered at 1500 hours0.0174 days <br />0.417 hours <br />0.00248 weeks <br />5.7075e-4 months <br />.

Operation is allowed for up to 30 days with one channel of heat tracing on the Boron Injection Tank or on the heat traced por-tion of an associated flow path operable provided the tank and flow path temperature are verified to be 145°F at least once per 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br />; otherwise be in hot shutdown within 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />.

The circuit was re-paired and the action statement was terminated at 0130 hours0.0015 days <br />0.0361 hours <br />2.149471e-4 weeks <br />4.9465e-5 months <br /> on February 28, 1980.

At 1500 on February 28, 1980, the action statement was entered a second time when circuit 601 primary became inoperable.

This circuit was repaired and the action statement was terminated at 2025 hours0.0234 days <br />0.563 hours <br />0.00335 weeks <br />7.705125e-4 months <br /> on February 28, 1980.

During the second failure the temperature of the system never went below 145°F.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The cause of both failures was dripping valves within the same area as the heat traced piping.

Boric acid solution dripped onto the piping, and after a period of time, the insulation was erroded away and the heat tracing became shorted.

ANALYSIS OF OCCURRENCE:

The circuits rendered inoperable were repaired and the pipes were reinsulated.

The valves are scheduled for repair at a future outage.

CORRECTIVE ACTION

The heat tapes were repaired and the piping has been reinsulated.

At a future outage the valves will be repaired.
