Missed Manipulator Crane Surveillance (T.S. 4.9.6.1)

CONDITIONS PRIOR TO OCCURRENCE:

Mode 6 prior to Fuel Movement DESCRIPTION OF OCCURRENCE:

While reviewing Maintenance Department Procedure M2F, Manipulator Crane Pre-Operational Inspection and Operational Tests, it was noted the specification for the load test was stated as 3250 + 0 -

50 pounds.

The Technical Specification surveillance requirement is for the crane to be tested within 100 hours0.00116 days <br />0.0278 hours <br />1.653439e-4 weeks <br />3.805e-5 months <br /> prior to the start of fuel movement at a test load of at least 3250 pounds.

A review of the surveillance performed prior to fuel movement on April 25, 1979, indicated the crane was tested to 3220 pounds, 30 pounds less than the test requirement.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The cause appears to be a typographical error when the procedure was originally typed.

The initial review did not find the error.

The procedure should have been written 3250 -

0 + 50 pounds, not 3250 + 0 50 pounds as was written.

ANALYSIS OF OCCURRENCE:

The surveillance requirement was met with regard to the manipulator crane being tested within 100 hours0.00116 days <br />0.0278 hours <br />1.653439e-4 weeks <br />3.805e-5 months <br /> of fuel movement, but the load test was not of a sufficient weight.

The refueling evolution has been completed without incident involving the manipulator crane.

CORRECTIVE ACTION

The procedure M2F is being revised to ensure a load test of at least 3250 pounds will be completed and will be used on all subsequent load tests.
