Report Number: 8~-12/03L Report Date: 3/3/80 Occurrence Date: 2/5/80 Facility: Salem Generating Station Public Service Electric & Gas Company Hancock's Bridge, New Jersey 08038 IDENTIFICATION OF OCCURRENCE: Failure of the Air Particulate Detector (T.S. 3.4.6.1) CONDITIONS PRIOR TO OCCURRENCE: Mode 1 Reactor Power 100% Unit Load 1140 MW DESCRIPTION OF OCCURRENCE:

• J A filter failure alarm was received at 1037 hours and the Action State-ment entered.

Investigation showed the shear pins used to connect the sampler movement drive to the coupling failed requiring replacement. The Action Statement (3.4.6.1) allows for continued operation up to 30 days if grab samples are taken at least every 24 hours on be in at least Hot Standby within the next 6 hours and in Cold Shutdown within the following 30 hours. When repairs were effected and the APD returned to service for testing, it was determined a micro switch had also failed. The mirco switch was unavailable, therefore, it was jumpered until permanent repairs could be made the following day. The Action Statement was terminated at 2200 hours on February 5, 1980 when the APD unit was returned to service. DESIGNATION OF APPARENT CAUSE OF OCCURRENCE: The cause was the failure of the shear pins as designed. The micro switch also failed, possibly due to personnel hitting it when in close proximity. ANALYSIS OF OCCURRENCE: The shear pins failed as designed caused by normal degradation of the components. The micro switch probably was bumped causing its failure. With the micro switch bypassed, an evaluation was made that the operability of the APD was not effected until permanent repairs were made.

LER 80-12/03L CORRECTIVE ACTION: The shear pins were replaced at once and the micro switch was repaired the following day (reference incident report 80-64). FAILURE DATA: Not Applicable
