Inoperable Containment Air Lock (TS 3~6.l.3b) 100' Elevation CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 Reactor Power 85%

Unit Load 960 mW DESCRIPTION OF OCCURRENCE:

At 1600 hours0.0185 days <br />0.444 hours <br />0.00265 weeks <br />6.088e-4 months <br /> on February 12, 1980, when performing an air lock leak-age rate test, it was determined the leak rate was > 0.05 La at 47 psig after an entry had been made into the containment.

The Action Statement 3.6.l.3(b) was entered which requires the air lock to be operable within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> or be in at least Hot Standby within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and Cold Shutdown within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

When repairs were effected, it was determined the seal had popped out of its seating channel when the door was slammed shut.

ANALYSIS OF OCCURRENCE:

The Action Statement was entered at 1600 hours0.0185 days <br />0.444 hours <br />0.00265 weeks <br />6.088e-4 months <br /> on February 12.

Upon investigation, the Maintenance Department supervisor determined a section of the air lock seal had popped from its channel.

He evaluated the situation and concluded it was caused by slamming the door shut.

It was repaired, tested satisfactorily and the Action Statement terminated at 1650 hours0.0191 days <br />0.458 hours <br />0.00273 weeks <br />6.27825e-4 months <br /> on February 12, 1980.

This occurrence is determined to be an isolated case.

CORRECTIVE ACTION

The channel and rubber gasket were cleaned and the gasket was repositioned in the channel.
