Failure of the Air Particulate Detector (T.S. 3.4.6.1)

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 Reactor Power 100%

Unit Load 1140 MW DESCRIPTION OF OCCURRENCE:

At 2100 hours0.0243 days <br />0.583 hours <br />0.00347 weeks <br />7.9905e-4 months <br /> on February 3, a low flow alarm was received for the air particulate detector (APD).

Investigation revealed the pump had seized.

Entered Action Statement 3.4.6.1 allowing operation for 30 days if grab samples are taken at least every 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> or be in at least Hot Standby within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and in Cold Shutdown within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The pump became seized.

ANALYSIS OF OCCURRENCE:

The pump was found to be seized when investigated by the Performance Department due to overheating.

It is felt the failure was due to normal degradation of the pump.

The pump was replaced, flow set and the motor current draw was determined to be satisfactory.

The Action Statement was terminated at 1350 hours0.0156 days <br />0.375 hours <br />0.00223 weeks <br />5.13675e-4 months <br /> on February 4, 1980.

CORRECTIVE ACTION

The pump was replaced and tested satisfactorily.
