Loss of Meteorological Tower Instrumentation (T.S. 3.3.3.4) Caused by A Loss of Power CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 Reactor Power 96%

Load 1095 MW DESCRIPTION OF OCCURRENCE:

At 1028 hours0.0119 days <br />0.286 hours <br />0.0017 weeks <br />3.91154e-4 months <br />, power was lost to the meteorological tower instruments when a phase to phase -short occurred in the lH 460V bus transformer.

Power was not immediately available from the meteorological tower emergency power supply due to a previous failure with the inverter transfer switch.

Power was restored at 0315 hours0.00365 days <br />0.0875 hours <br />5.208333e-4 weeks <br />1.198575e-4 months <br /> on January 24, 1980, and the Action Statement was terminated.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The meteorological tower inverter was not immediately available because a new inverter that was installed did not have an automatic transfer switch.

Upon the loss of power to the lH 460V bus, power was lost until the lH bus was re-energized *

.. ANALYSIS OF OCCURRENCE:

In November 1979, a short occurred in the meteorolgocal tower and damaged the emergency power inverter beyond repair (LER 79-72/03L)

  • A new inverter was ordered and received but did not come with an automatic transfer switch since they are no longer manufactured with that feature.

An investigation is being conducted to either obtain an inverter equipped with the automatic transfer switch or an automatic tra.nsfer switch that would be compatible with the present inverter.

The present inverter does have a manual switch to allow the use of the emergency power supply but requires an operator to make that transfer at the meteorological tower.

LEE. 80-06/03L CORRECTIVE ACTION:

Power was restored to the instrumentation at 0315 hours0.00365 days <br />0.0875 hours <br />5.208333e-4 weeks <br />1.198575e-4 months <br />, January 24, 1980, by re-energizing the lH 460V bus.

Power could have been restored quicker by manually transferring the power supply to the emergency power source.

It was decided to wait until power was again available from the lH bus to preclude the necessity to de-energize the instruments a second time.

An investigation is in progress to determine if a different inverter can be used with an automatic transfer switch or if an automatic transfer switch can be adapted to the inverter presently installed.
