Exceeding Axial Flux Difference Target Band CONDITIONS PRIOR TO OCCURRENCE:

Unit in Mode 1 reactor power at 40%, producing 350 mW DESCRIPTION OF OCCURRENCE:

At 1320 hours0.0153 days <br />0.367 hours <br />0.00218 weeks <br />5.0226e-4 months <br />, the axial flux difference (AFD) was determined to be outside its target band,+ 5% of target value (T.S. 3.2.1).

Initial investigation indicated the summing unit had failed.

The AFD monitor was declared inoperable and power secured to the unit.

Instrument and controls personnel were summoned to effect repairs.

At the start of the troubleshooting process, the unit was re-energized and appeared to function properly.

It was declared operable at 1450 hours0.0168 days <br />0.403 hours <br />0.0024 weeks <br />5.51725e-4 months <br />.

At 1655 hours0.0192 days <br />0.46 hours <br />0.00274 weeks <br />6.297275e-4 months <br />, the same symptoms appeared and the unit was declared inoperable a second time.

Subsequent troubleshooting identified the failed component to be the signal isolator.

The signal isolator separates the loop 13 6T Tavg signal from the AFD monitor.

After replacement and recalibration, the monitor was declared operable at 1835 hours0.0212 days <br />0.51 hours <br />0.00303 weeks <br />6.982175e-4 months <br />.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The signal isolator failed.

ANALYSIS OF OCCURRENCE:

Since reactor power was only 40%, no adverse conditions existed.

Reactor power was maintained to continue the physics testing program at 40% power.

CORRECTIVE ACTION

The signal isolator was replaced in kind and the AFD monitor was calibrated.
