Partial Collapse of 12 CVCS Holdup Tank CONDITIONS PRIOR TO OCCURRENCE:

At 1400 hours0.0162 days <br />0.389 hours <br />0.00231 weeks <br />5.327e-4 months <br /> on 12/19/79, 11 eves Holdup Tank (HUT) was placed on recirculation and at 1720 hours0.0199 days <br />0.478 hours <br />0.00284 weeks <br />6.5446e-4 months <br />, the tank was sampled for boron.

Late on the 1600 to 2400 hour0.0278 days <br />0.667 hours <br />0.00397 weeks <br />9.132e-4 months <br /> shift, 11 eves HUT was taken off of recirculation and 12 eves HUT was placed on recirculation.

Tank levels at this time were 11 eves HUT -

92%, 12 eves HUT -

84% and 13 CVCS HUT -

31%..

At 0015 hours1.736111e-4 days <br />0.00417 hours <br />2.480159e-5 weeks <br />5.7075e-6 months <br />, 12/20/79, the Equipment Operator (EO) in the Auxiliary Building reported both gas compressors running and a vent header pressure of 7 psig.

A check of the valve lineup showed 1BR78 positioned to 11 CVCS HUT.

With 1BR78 in this position, the recirculation pump taking suction from 12 eves HUT was discharging to 11 eves HUT.

Tank levels at this time were 11 eves HUT -

100%,

12 eves iliUT -

60% and 13 eves HUT -

44%.

1BR78 was repositioned to discharge flow to 12 CVCS HUT and the vent header pressure decreased to 4 psig.

The gas compressors tripped off and when the com~ressor moisture separator tanks were checked, both tanks indicated ~JO%.

The EO was instructed to pump from 11 CVCS HUT to 12 CVCS HU~ until the level in 11 CVeS HUT decreased to less than 100%.

When the recirculation pump was started, a noise was heard from the vicinity of 12 eves HUT.

The pump was stopped and 12 eves HUT was found with a dished in area approximately 3 feet deep along the length of the tank about 30 degrees off centerline and several smaller indentations around the tank.

The valve lineup was checked and found satisfactory.

Air surveys taken in the tank area showed normal activity.

11 and 12 CVCS HUT's were drained to 12 Waste Holdup Tank to establish positive tank levels.

Positive tank pressure was maintained on both tanks while draining.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The cause of this occurrence was personnel error.

When 11 eves HUT was taken off of recirculation and 12 eves HUT was placed on r~circu lation, the recirculation pump discharge was not switched to 12 eves HUT.

Consequently, water was pumped from 12 CVCS HUT to 11 CVCS HUT.

LER 79-76/03L When 11 eves HUT became full, water was forced through the common vent header to 12 and 13 eves HUT's.

With water in the vent header, 12 eves HUT was unable to equalize pressure resulting in the tank indentations.

The event investigation concluded that the noise heard at 0015 hours1.736111e-4 days <br />0.00417 hours <br />2.480159e-5 weeks <br />5.7075e-6 months <br /> was probably the partial reforming of the tank when the system was realigned to pump water back into 12 eves HUT.

The eves Holdup Tanks are part of the recycle portion of the eve System.

These tanks receive borated radioactive letdown and clean borated radioactive drains for processing or disposal.

The deformation of the 12 eves HUT did not cause any leakage of liquid or gas from the tank and did not constitute a hazard to the health and safety of the general public.

CORRECTIVE ACTION

A design change has been initiated to structurally re-enforce the eves Holdup Tanks.

This modification has been accomplished on 13 eves HUT and will be included in the main~enance action for 11 and 12 eves HUT's.

A design change to install vacuum breakers for each of the tanks has been approved and will be done during normal plant operations by removing one tank at a time from service and performing the modification.
