79-67/03L 10/17/79 Salem Generating Station Public Service Electric & Gas Hancock's Bridge, New Jersey IDENTIFI~ATION OF OCCURRENCE:

Depressed Areas on llCVC Holdup Tank CONDITIONS PRIOR TO OCCURRENCE:

Operational Mode 5 DESCRIPTION OF OCCURRENCE:

Company 08038 At 1300 hours0.015 days <br />0.361 hours <br />0.00215 weeks <br />4.9465e-4 months <br />, the Boric Acid Evaporator was placed in service on llCVC Holdup Tank.

At 1509 hours0.0175 days <br />0.419 hours <br />0.0025 weeks <br />5.741745e-4 months <br />, the primary plant operator on elevation 64 of the Auxiliary Building reported a noise and dust in the vicinity of the eve Holdup Tanks.

As a precautionary measure, Auxiliary Building elevation 64 was evacuated, the APD unit was shifted to the plant vent and the building HEPA and charcoal filters were placed in service.

Health Physics personnel took air surveys of the area.

Investigation showed that llCVC Holdup Tank had a depressed crease on top of the tank and several other areas dented.

The damage to the tank did not cause any leaks of liquid or vapor frorn the tank.

At 1555 hours0.018 days <br />0.432 hours <br />0.00257 weeks <br />5.916775e-4 months <br />, with normal readings on the radiation monitors, the APD unit was returned to the containment mode and the charcoal filter was secured.

Further investigation showed that the valve connecting llCVC HUT to the vent header was shut.

The vent valve was opened and processing of the tank contents resumed.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Investigation of this incident could not determine who had shut the holdup tank vent valve.

The normal system valve lineup shows this valve locked open.

There were no remarks on the valve deviation sheet which indicated the valve abnormal position.

ANALYSIS OF OCCURRENCE:

The eve Holdup Tanks are each inside concrete dikes. which will contain the volume of a full holdup tank if a leak were to occur.

The damage to the tank did not result in liquid or vapor release to the building atmosphere1 Samples taken on elevation 64 showed airborne activity of 2.3xl0- O µc/cc.

LER 79-67/03L CORRECTIVE ACTION:

A design change has been initiated which will provide vacuum breakers for the eves tanks.

A station Manager letter was recently routed to all personnel on-site which re-emphasized the station policy of only Operations Department personnel operating system valves in accordance with established procedures.

The Company response to NRC letter dated 11-1-79 to Mr. F. W. Schneider will present the administrative controls established to maintain proper valve alignment.
