Low Pressure co2 Storage Tank Level Less Than Required by Technical Specification CONDITIONS PRIOR TO OCCURRENCE:

Operational Mode 5 DESCRIPTION OF OCCURRENCE:

During a review of plant conditions for a shift returning from days off, the relieving Senior Shift Supervisor noted that the Low Pressure C02 Storage Tank level was indicated on the primary log as being 45%.

A review of the applicable Technical Specification showed that with the storage tank level less than 50%, fire watches of unprotected areas shall be established with backup fire sup-pression equipment.

The Action Statement for Technical Specification 3.7.10.3 was initiated and fire watches were posted.

At 1545 hours0.0179 days <br />0.429 hours <br />0.00255 weeks <br />5.878725e-4 months <br /> on 10-2-79, the C02 Storage Tank had been recharged and the Action Statement was terminated.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The cause of this occurrence was personnel error.

On 9-26-79, two heating water high point vent valves on the room heaters of B and C diesel generator rooms were left open while the system was being filled and vented.

The resulting heat buildup in these rooms caused the heat detectors to initiate C02 discharge to the B&C diesel generator rooms and decrease the storage tank level to 45%.

On shift personnel did not consider the C02 fire suppression system to b~ inoperable and no fire watches were posted.

This condition was not corrected until questioned by the relieving Senior Shift Supervisor on 10-1-79.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.7.10.3 requires the low pressure C02 systems to be operable with a minimum level of 50% and a minimum pressure of 285 psig in the associated storage tanks.

With one or more of the required low pressure C02 systems inoperable, establish a continuous fire watch with backup fire suppression equipment for

LER 79-64/0lT the unprotected areas within one hour; restore the system to operable status within 14 days or submit a report to the Commission.

The C02 fire suppression storage tank is sized to provide two C02 actuations to the largest protected area at 55% capacity.

All protected areas are equipped with smoke and heat detectors which alarm in the control room.

Besides the alarm, the diesel generator area monitors provide automatic C02 initiation.

With 45% level in the storage tank and all detectors and alarms operable, all required areas are fully protected.

Backup station fire suppression systems are available at all areas with sufficient fire brigade personnel available to respond to fire alarms 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> a day.

CORRECTIVE ACTION

A change to the Primary Plant Log No. 6 has been completec to change the minimum C02 storage tank level from 50%. to 75% and to provide a notation to flag the fact that a minimum 50% is a Technical Specif i-cation requirement.
