Open Electrical Penetration Fire Barrier with No Fire Watch CONDITIONS PRIOR TO OCCURRENCE:

Operational Mode 5 DESCRIPTION OF OCCURRENCE:

During a *station Q.A. surveillance of the Auxiliary Building, two four inch conduits penetrating the 84' elevation floor were found not sealed or capped and no fire watch posted.

This incident was immediately reported to the Senior Shift Supervisor and the maintenance contractor, who had last notified the Shift Supervisor of work performed on the penetrations on 9-5-79.

The maintenance contractor inspected the conduits and had both ends of the con-duits sealed properly in accordance with Maintenance Procedure M3Y.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

It appears that this incident was due to a misinterpretation of Maintenance Procedure M3Y for repair of fire and flood protection seals.

The conduits were installed through the floor on 9-5-79 and properly sealed.

During the day shifts on 9-6-79 and 9-7-79, the conduits were extended approximately 10 feet above elevation 84 and approximately 24 feet was added below elevation 84.

The supervisor in charge of the work failed to note that the fire barriers would be broken during this work and approval must be obtained in the same manner as broaching any other seal.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.7.11 requires that with one or more of the penetration fire barriers protecting safety related areas non-functional, establish a continuous fire watch on at least one side of the affected penetration within one hour.

The conduit fire barriers were inoperable from 9-6-79 to 0830 hours0.00961 days <br />0.231 hours <br />0.00137 weeks <br />3.15815e-4 months <br /> on 9-10-79 with no fire watch posted.

LER 79-61/03L CORRECTIVE ACTION:

A memorandum has been issued to all maintenance contractor personnel clarifying the requirements of Maintenance Procedure M3Y.

All supervisors and craft foremen, who may become involved in breaking fire barriers, have been reinstructed in the requirements of the fire and flood barrier procedure.
