Loss of Safeguards Manual Initiation/Reset Functions CONDITIONS PRIOR TO OCCURRENCE:

Operational Mode 5 DESCRIPTION OF OCCURRENCE:

During performance of the 18 month surveillance of manual initiation of safety injection and the radiation monitoring system containment isolation functional test, several safeguards systems could not be manually reset due to the 28 volt DC control power circuit breakers being tripped.

Performance Department investigated the problem and has initiated corrective action.

This incident does not affect the automatic operation of the safeguard systems under any operating or shutdown conditions.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The cause of this occurrence is a design deficiency in the 28 volt DC control power circuit which supplies console indication and initiation/reset functions for console operations.

The circuit breakers are rated for one amp continuous load; but the circuits fed from these breakers have a total load of approximately 2.5 amps.

Under normal operating conditions, the operating load is less than the trip settings of the breakers; but under certain shutdown con-ditions, more indicating and control circuits are energized and simultaneous energizing of a group of relays, solenoids, etc. can cause a breaker overload condition.

ANALYSIS OF OCCURRENCE:

The safeguards systems which are affected by the tripped circuit breakers are not required to be operational during Mode 5 and no Technical Specifications were violated.

The conditions which cause the overload to occur are not present during normal plant operations.

Functional tests of these systems in required modes have verified operability.

LER 79-60/03L CORRECTIVE ACTION:

A DCR (PD-0153) has been initiated which will replace the existing 1 amp circuit breakers with breakers of 2 amp continuous load rating.

The DCR has been approved and the replacement breakers will be installed prior to entry into Mode 4.
