!The cause of this occurrence was personnel error in that the deficient C2:IIJ /penetrations were not noted during the November inspection.

The pene-IIIIlltrations were properly sealed and all sealant on site verified to be of OJ)) !the approved type.

Administrative controls are adequate to prevent o:ITJ I reoccurrence of this type of incident during plant operation..
