Fire Barriers Inoperable CONDITIONS PRIOR TO OCCURRENCE:

Mode VI, Refueling DESCRIPTION OF OCCURRENCE:

On 5/17/79, a QA Specialist and the Safety Supervisor conducted the 18 month surveillance inspection of penetration fire barriers as required by T~chnical Specification 4.7.11.

One penetration was found open, no fire barrier material installed.

Three other penetrations were found to be inadequately sealed.

At 1530 hours0.0177 days <br />0.425 hours <br />0.00253 weeks <br />5.82165e-4 months <br /> on 5/17/79, Action Statement 3.7.11 was implemented and a fire watch was established.

At 0930 hours0.0108 days <br />0.258 hours <br />0.00154 weeks <br />3.53865e-4 months <br /> on 5/18/79, the fire barriers had been sealed and returned to a func-tional status.

Action Statement 3.7.11 was terrainated.

DESIGNATIDN OF APPARENT CAUSE OF OCCURH.ENCE:

The cause of this occurrence is. personnel error in that these penetrations were not noted as having deficient fire barriers during a penetration

  • inspection conducted by the Maintenance Departn1ent in November, 1978.

The inspection which discovered the deficient fire barriers is the first of the Technical Specification 18 month surveillances to be performed.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.7.11, Action Statement a, states that with one or more of the required penetration fire barriers non-functional, establish a continuous fire watch on at least one side of the affected penetration within one hour.

Upon discovery of the non-functional fire barriers, a fire watch was stationed and the. fire barriers were repaired.

CORRECTIVE ACTION

The deficient penetrations were properly sealed with an approved sealant.

The Maintenance Department, maintenance contractor and building contractor

LER 79-42/03L procedures for penetration sealing were reviewed and verified to be satisfactory.

These penetrations must not have been properly sealed during the construction phase of the station.

Our maintenance procedures*

and surveillances are adequate to prevent this type of occurrence during the operation of the plant.
