Occurrence Date:
Facility:
79-34/03L 5/1/79 3/31/79 Salem Generating Station Public Service Electric & Gas Company Hancock's Bridge, New Jersey 08038 IDENTIFICATION OF OCCURRENCE:
Stearnline Differential Pressure Loop 12 Inoperable
- CONDITIONS PRIOR TO OCCURRENCE:
Operational Mode 1 Reactor Power 100%
DESCRIPTION OF OCCURRENCE:
At 0457 hours0.00529 days
0.127 hours
7.556217e-4 weeks
1.738885e-4 months
, overhead annunciators and status panel alarms in the Control Room indicated steamline differential pressure loop 12 greater
- than 100 psig lower than loop 11.
Steamline pressure indication for loop 11 and 12 showed this to be an invalid alarm.
At 0526 hours0.00609 days
0.146 hours
8.69709e-4 weeks
2.00143e-4 months
, the differential channel was declared inoperable and channel bistables were placed in the tripped condition.
Technical Specification 3.3.2.1.b Table 3.3-3 Action 14 was implemented.
The channel was repaired and returned to service at 1150 hours0.0133 days
0.319 hours
0.0019 weeks
4.37575e-4 months
.
The T/S Action.Statement was ter-minated at this time.
DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:
The cause of this occurrence was a failed comparator module which com-pares the loop 11 and loop 12 stearnline pressures and transmits a differential output.
ANALYSIS OF OCCURRENCE:
Technical Specification 3.3.2.1.b Table 3.3-3 Action 14 requires that with the number of operable channels one less than the total number of channels, operation may proceed until performance of the next required channel functional test, provided the inoperable channel is placed in the tripped condition within one hour.
The channel bistables were placed in the tripped condition within the required one hour and the channel was repaired, tested and returned to service within seven hours.
All other steamline differential, channels were operable throughout this occurrence.
CORRECTIVE ACTION
The failed comparator was replaced with a spare module.
The comparator was calibrated and correct operation of channel setpoints were verified.
Repair of the failed comparator is being followed on Deficiency Report PD-754.
