Containment Personnel Air Lock Elevation 100 Inoperable CONDITIONS PRIOR TO OCCURRENCE:

Operational Mode 1 Reactor Power 100%

DESCRIPTION OF OCCURRENCE:

At 0930 hours0.0108 days <br />0.258 hours <br />0.00154 weeks <br />3.53865e-4 months <br />, the containment personnel air lock elevation 100 failed to pass a leak rate test.

Technical Specification 3.6.1.3.b Action Statement was implemented.

Maintenance Department personnel repaired the inner door and surveillance 4.6.1.3 was performed to verify opera-bility.

The Action Statement was terminated at 1500 hours0.0174 days <br />0.417 hours <br />0.00248 weeks <br />5.7075e-4 months <br />.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The caus~ of this occurrence was a split inner door inner seal which appeared to have been snagged by some object passing through the door.

There was no other damage to the door sealing surface or operating mechanism.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.6.1.3 requires that with an air lock inoper-able, restore the air lock to operable status within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> or be in at least hot standby within the next six hours and in cold shutdown within th~ following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

The air lock was repaired and returned to service within 5 hours5.787037e-5 days <br />0.00139 hours <br />8.267196e-6 weeks <br />1.9025e-6 months <br /> and 30 minutes.

CORRECTIVE ACTION

The air lock inner door inner seal was replaced and the door was adjusted for a proper seal.

A satisfactory leak rate test was per-formed before returning the air lock to service.
