RMS Channel 1Rl2B Inoperable CONDITIONS PRIOR TO OCCURRENCE:

Operational Mode 1 Reactor Power 100%

DESCRIPTION OF OCCURRENCE:

At 1020 hours0.0118 days <br />0.283 hours <br />0.00169 weeks <br />3.8811e-4 months <br />, channel functional tests of RMS channels lRllA, 1Rl2A, and 1Rl2B were started and Action Statement 22 for Technical Speci-fication 3.3.3.lb was implemented.

The tests showed that the 1Rl2B channel alarm was not functioning properly.

Trouble-shooting channel components corrected the problem and the channel was declared operable at 1215 hours0.0141 days <br />0.338 hours <br />0.00201 weeks <br />4.623075e-4 months <br /> on 2-21-79.

The Action Statement for T/S 3.3.3.lb was terminated.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The cause of this occurrence is unknown.

Trouble-shooting of the channel did not identify any component failure.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.3.3.lb Action 22 requires that with the number of channels operable less than required by the minimum channels operable requirement, comply with the action requirement of Specification 3.9.9 which requires that with the containment purge and pressure-vacuum relief isolation system inoperable, close each of the purge and pressure-vacuum relief penetrations providing direct access from the containment atmosphere to the outside atmos-phere.

The containment purge and pressure-vacuum relief isolation system is operable if isolation occurs on manual initiation and on a high radiation test signal from each of the containment RMS channels.

The applicable penetrations were closed at the time 1Rl2B alarm failure1was detected and remained closed in accordance with the Action Statement until the channel was returned to operable status at 1215 on 2-21-79.

CORRECTIVE ACTION

The channel connectors and the alarm relays were inspected and tested satisfactory.

Subsequent performance of the channel functional test showed the channel to be operating properly.

No further corrective action is required.
