No. 14 Containment Fan Coil Unit Inoperable CONDITIONS PRIOR TO OCCURRENCE:

Operational Mode 1 Reactor Power 100%

DESCRIPTION OF OCCURRENCE:

At 1842 hours0.0213 days <br />0.512 hours <br />0.00305 weeks <br />7.00881e-4 months <br />, No. 14 containment fan coil unit was started in slow speed and secured three minutes later due to no indication of service water flow.

Performance personnel were notified and tests performed showed no service water flow in either fast or slow speed.

The fan coil unit was declared inoperable and the Action Statement for Technical Specification 3.6.2.3 was implemented.

Rework of the service water flow control valve and retests to verify service water flow were completed at 0020, 2-21-79, and the Action Statement was terminated.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The cause of this occurrence was a burned out coil in solenoid valve SV-630 which supplies either closing air or flow control air to 14 SW223, the service water flow control valve for No. 14 C.F.C.U.

SV630 was found inoperable in a position which kept 14 SW223 shut.

The coil is believed to have burned out during the slow speed start at 1842 hours0.0213 days <br />0.512 hours <br />0.00305 weeks <br />7.00881e-4 months <br />.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.6.2.3 Action Statement requires that with one group of containment cooling fans inoperable, restore the inoperable fan group to operable status within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> or be in hot shutdown within the next 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />.

No. 14 C.F.C.U. was returned to service within six hours.

No. 11, 12, 13 and 15 C.F.C. units were available throughout this occurrence.

CORRECTIVE ACTION

The burned out coil on SV-630 was replaced and Surveillance Procedure SP(O) 4.6.2.3.A was performed to verify operability of the fan coil unit and service water flow indication.
