'-07/03L Report Date:
02-23-79 Occurrence Date:
1-26-79 Facility:
Salem Generating Station Public Service Electric & Gas Company Hancock's Bridge, New Jersey 08038 IDENTIFICATION OF OCCURRENCE:
Pressurizer Level Channel II Inoperable CONDITIONS PRIOR TO OCCURRENCE:
Operational Mode 1 Reactor Power 70%
DESCRIPTION OF OCCURRENCE:
At 1310 hours0.0152 days
0.364 hours
0.00217 weeks
4.98455e-4 months
, while performing channel checks of pressurizer level, Channel II was found to be indicating approximately 5% lower than other channels.
Technical Specifications 3.3.1.1, Action 7, and 3.3.2.1, Action 14, were implemented.
The channel bistables were placed in the tripped condition.
The channel was returned to service at 1000 hours0.0116 days
0.278 hours
0.00165 weeks
3.805e-4 months
.
on 2-3-79 and the Action Statements were terminated.
DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:
The cause of this occurrence was transmitter drift which resulted in a zero shift in the transmitter output.
ANALYSIS OF OCCURRENCE:
Actions 7 and 14 for Technical Specifications 3.3.1.1 and 3.3.2.l permit operation until the next required channel functional test with the num-ber of operable channels one less than the total number of channels provided the inoperable channel is placed in the tripped condition within one hour.
Pressurizer lev.el Channel II was placed in the tripped con-dition within one hour and remained in this condition until the channel was returned to operable status.
Pressurizer level Channels I and IV were operable throughout this occurrence.
CORRECTIVE ACTION
The pressurizer level Channel II transmitter was recalibrated in place in accordance with Performance Department procedure.
A channel functional test was performed to verify operability.
