Operational Mode 1 Reactor Power.91%

DESCRIPTION OF OCCURRENCE:

At 1630 hours0.0189 days <br />0.453 hours <br />0.0027 weeks <br />6.20215e-4 months <br />, No. 11 Steam Generator steam flow Chann'el II, which was.

the controlling channel for No. 11 Steam Generator level control, failed low.

The Control Operator took manual control and re-established S/G level.

Action 7 for Technical Specification 3.3.1.1 and Action 14 for T/S 3.3.2.1 were implemented.

Channel bistables were placed in the tripped condition.

The channel indication returned to normal within ten minutes of failing low and indicated properly throughout the remainder of the occurrence.

At 1930 hours0.0223 days <br />0.536 hours <br />0.00319 weeks <br />7.34365e-4 months <br /> on 1-16-79, the channel was returned to service and the T/S Action Statements were terminated.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The cause of this occurren.ce could not be determined.

All tests con-ducted showed the channel to be operating properly.

ANALYSIS OF OCCURRENCE:

The Action Statements for Technical Specificat~ons 3.3.l.l and 3.3.2.l allow continued operation until performance of the next required Channel Functional Test, provided the inoperable channel is placed in the tripped condition within one hour.

The inoperable channel was tripped and remained in the tripped condition throughout this occurrence.

No. 11 S/G steam flow Channel I was operable and provided steam flow protection during this time.

Channel II was returned to operable status within 27 hours3.125e-4 days <br />0.0075 hours <br />4.464286e-5 weeks <br />1.02735e-5 months <br />.

LER 79-12/03L CORRECTIVE ACTION:

Troubleshooting was performed consisting of applying test currents to the channel over the range of channel operation and monitoring channel response.

Internal and external connections were inspected.

A brush recorder connected to record the steam flow input to the channel was monitored over a 72 hour8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> period.* No problem areas could be identified.
