Containment Internal Pressure Exceeded Technical Specification Limit CONDITIONS PRIOR TO OCCURRENCE:

DESCRIPTION OF OCCURRENCE:

  • At 0830 hours0.00961 days <br />0.231 hours <br />0.00137 weeks <br />3.15815e-4 months <br /> on 1-3-79,. containment isolation valve 1VC6 was removed from service for rework.

The Action Statement for Technical Specif ica-tion 3.6.3.1 was implemented and the containment pressure/vaculim relief penetration was isolated as required by deactivating lVCS in the closed position.

The Containment Fan Coil Units (CFCU) were operated in slow speed as necessary to control containment pressure.

At 0551 on 1-6-79, with 5 CFCU's in low speed, containment pressure exceeded the 0.3 psig Technical Specification limit.* The Action Statement for T/S 3.6.1.4 was implemented.

Isolation valve 1VC5 was reactivated under adminis-trative control, and at 0630, a pressure relief was started.

The Action Statement for T/S 3.6.1.4 was terminated at 0745 hours0.00862 days <br />0.207 hours <br />0.00123 weeks <br />2.834725e-4 months <br /> with containment pressure at 0.25 psig.

DESIGNATION OF ~PPARENT CAUSE OF OCCURRENCE:

The cause of this occurrence was due to isolation of the containment pressure/vacuum relief system for maintenance of 1VC6~

ANALYSIS OF OCCURRENCE:

Technical Specification 3.6.1.4 requires that with the containment internal pressure outside.of the limits, restore the internal pressure to within the limits within 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br /> or be in at least hot standby within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and in cold shutdown within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

Internal pressure was reduced below the alarm point of 0.25 in 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br /> 54 minutes.

  • A power reduction was in progress throughout this occur-rence.

CORRECTIVE ACTION

The Chief Engineer's permission was received to open Ives for the pressure relief.

lVCS was shut and retagged after the pressure relief was completed.

The pressure/vacuum relief system was tested satisfac-tory on 1-7-79 when rework was completed on 1VC6.
