Tree, on a YELLOW condition.

NOT OBTAINED CAUTION STEAM RELEASES FROM AFFECTED S/G(S)

SHOULD BE MINIMIZED.

NOTE:

o Throughout this procedure "affected" refers to any S/G in which narrow range level is less than 5X [25X adverse CNMT].

o Adverse CNMT values should be used whenever CNMT pressure is greater than 4 psig or CNMT radiation is greater than 10

~ R/hr.

1 Identify Affected S/G(s):

a.

Narrow range level - LESS THAN 5X [25X adverse CNMT]

a. Return to procedure and step in effect.

2 Verify S/G Blowdown Isolation Valves From Affected S/G(s)

CLOSED

~ AOV-5738 for S/G A

~ AOV-5737 for S/G B

Place S/G blowdown valves master switch to CLOSE.

IF valves can NOT be closed

manually, THEN dispatch AO to locally isolate blowdown.

o S/G A, close V-5701 o

S/G B, close V-5702

NOT OBTAINED CAUTION AFV FLOV SHOULD NOT ESTABLISHED TO ANY FAULTED S/G THAT HAS BEEN ISOLATED UNLESS NEEDED POR RCS COOLDOVN.

3 Check If Affected S/G(s)

Secondary Side Is Intact:

o Pressure in affected S/G(s)-

STABLE OR INCREASING o

Pressure in affected S/G(s)-

GREATER THAN 100 PSIG IP affected S/G(s) previously identified as faulted, THEN return to procedure and step in effect.

IP affected S/G pressure decreasing in an uncontrolled manner or completely depressurized and has not been previously isolated, THEN go to E-2, FAULTED STEAM GENERATOR ISOLATION, Step 1.

4 Check AFW Flow To Affected S/G(s)

GREATER THAN 50 GPM IF affected S/G(s) wide range level greater than 35 inches

[110 inches adverse CNMT], THEN establish AFV flow as necessary to refill affected S/G(s).

IF affected S/G(s) wide range level less than 35 inches

[110 inches adverse CNMT], THEN establish APV flow to affected S/G(s) at a rate not to exceed 100 gpm.

5 Continue Filling Affected S/G(s) Until Narrow Range Level Greater Than 54

[254 adverse CNMT]
