ATTACHMENT l It is alleged that:
Two p5eces of wood were discovered in the 5ntake structure concrete wa11.
One piece of wood was removed and the hole grouted.
There are numerous deficient concrete pours at the 5ntake structure.
(Stokes ll/17/83 Aff. at 14.)
1.
On July 28, 1983, taro small pieces of wood were found enhedded 5n the concrete at the Intake Structure when Pullman was drilling holes for concrete anchor bolts for a pipe hanger on the center wall between the Auxiliary Saltwater Pumps.
The follow(ng steps were taken when the wood was discovered:
A PGandE Civil Engineering representative had the H. P. Foley Company chip out the wood and two small pieces of wood were removed from the area.
An area of 6 to 10 5nches 5n diameter was chipped out to a depth of less than 3 5nches.
Reinforcing steel was not exposed and the concrete was found to be sound.
Therefore, 5t was determined that repair of the area was of a cosmetic nature.
The concrete surface repairs were'erformed by H. P. Foley.
On December 1, 1983, a sample test program using a Schmidt Concrete Test Hanaer was performed.
The purpose of the test was to compare and verify the concrete strengths 5n areas around the above noted hanger and adjacent walls.
Results showed all concrete tested exceeded design strengths.
On December 5, 1983, a compar5son of the actual concrete strengths was perforaed using a Schmidt Concrete Test Hamner and a Fortney Test Machine.
A coepar5son of the test results 5ndicated that the Schmidt Test Haneer results were running approximately 11% low.
Therefore, the strength of the concrete tested at the Intake Struc..re on December 1, 1983, was greater than 5ndicated by the Sche5dt Test.
ATTACHMENT.1 I'.
The statement that 'there are numerous deffcfent concrete pour s at the Intake Structure'otally fs false.
The qualfty of the concrete ftself was assured by thorough testfng performed at the PGandE concrete testing
'I laboratory.
Iatchfng records and concrete test reports have been etfntafned.
Concrete afx pr opm tfons ~e approved by NandE's Department of Engfneerfng
- Research, and PGandE approval was requfred for all lfftdrawfngs and for all lffts prfor to concrete placement.
PGandE General Constructfon cfvf1 fnspectors observed all concrete placements providfng oversight and assurfng qualfty work.
