CLOSURE OF INVESTIGATION (01 4-2014-005)

Dear Mr. Mulligan :

This refers to an investigation conducted by the NRC's Office of Investigations regarding activities at Grand Gulf Nuclear Station . The Office of Investigations initiated the investigation to determine whether a former security operations supervisor at Entergy Operations, lnc.'s Grand Gulf Nuclear Station willfully falsified information on a fatigue assessment form , in an attempt to have a security officer sign the form , and also to determine if Entergy Operations , lnc.'s employees willfully provided incomplete and inaccurate information in response to an NRC request for information.

Based on the evidence developed during the investigation , the allegation that a security operations supervisor at Grand Gulf Nuclear Station willfully falsified information on fatigue assessment forms was not substantiated.

Based on the evidence developed during this investigation , the allegation that Entergy Operations, lnc.'s employees at Grand Gulf Nuclear Station willfully provided incomplete and inaccurate information in response to an NRC request for information was not substantiated .

The NRC plans no further action related to the willful aspects of these allegations and considers this investigation closed .
