21.21(d)(3)(i) DEFECTS AND NONCOMPLIANCE PART 21 - TYPE 67 AFR REGULATORS WITH IMPROPERLY DEDICATED DIAPHRAGM VALVE SEATS The following information was summarized from the report obtained from the vendor via facsimile:

"The purpose of this Fisher Information Notice is to alert CPNPP [Comanche Peak Nuclear Power Plant], that as of October 6, 2014, Fisher Controls International LLC (Fisher) became aware of a situation which may affect the performance of the Equipment, including their safety-related function.

"Fisher is informing CPNPP of this circumstance in accordance with Section 21.21 (b) of 10 CFR 21.

"Fisher has determined that parts kits for the Equipment were provided with improperly dedicated parts that had not been properly processed per active valve requirements. Specifically, Fisher provided diaphragm assemblies for the Type 67 AFR regulator with improperly dedicated diaphragm valve seats."

Items subject to this FisherInformation Notice (FIN) were provided to Comanche Peak Nuclear Power Plant (CPNPP) via FisherOrderNumbers 0270-402411, 027D-143252, 027LO-154408, 027D-174939, 027D-180940, 027D-183925, and 027D-182700. A total of 267 retrofit kits (FSCCOLOO527) were delivered via the aforementionedOrders for the subject regulators, In each kit, the improperly dedicatedpart Type 67R DiaphragmAssembly 19A7667X182 was identified. (Equipment).

Purpose:

The purpose of this FIN is to alert CPNPP that as of 6 October 2014, Fisher Controls International LLC (Fisher) became aware of a situation which may affect the performance of the Equipment, including their safety-related function-Fisher is informing CPNPP of this circumstance in accordance with Section 21.21 (b) of 10 CFR 21.

This FIN applies only to the subject Equipment supplied by Fisher Controls to CPNPP via the aforementioned orders.

Discussion:

Fisher has determined that parts kits for the Equipment were provided with improperly dedicated parts that had not been properly processed per active valve requirements. Specifically, Fisher provided diaphragm assemblies for the Type 67AFR regulator with improperly dedicated diaphragm valve seats.

Concerning Fisher order 027D-402411, all 9 kits supplied were returned and evaluated. Itwas determined that an out of tolerance condition exists. This nonconformance, when present, will prevent the assembled regulator from functioning as was determined by CPNPP upon testing after assembly. Itis assumed that any kits supplied on previous orders that are in service have successfully undergone the same functional testing do not have the same defect.

Action Required:

Fisher will support CPNPP in performing the replacement of the now obsolete 67AFR regulators with the current offering of 67CFR regulators. A credit will be issued for the full cost of the nine kits that were supplied for Fisher order 0271-402411. Additionally, any unused kits from aforementioned orders that remain in stock should be returned to Fisher and a credit will be issued for any returned kits as well. Fisher will further support CPNPP with information necessary to assist in performing engineering justification for the change from the 67AFR to the 67CFR regulator. Arrangements will be made with CPNPP in order to obtain replacement regulators.

Inaddition, a Corrective Action Request (CAR 2014-014) has been initiated by Fisher to prevent reoccurrence of this issue.

Please note that Fisher will no longer offer replacement parts or conversion kits for the obsolete 67AFR regulator.
