Dear Mr. Stello:

Re: Turkey Point Unit 4 Docket No. 50-251 Proposed Amendment to Facility Operating License DPR-41 This letter augments Florida Power 6 Light's letter of July 6, 1977, (L-77-208) which requested a license amendment authorizing operation of the facility for six months at, which time the steam generators would be reinspected, and also supplements the infor-mation forwarded to Mr. George Lear of your staff on July 27, 1977, (L-77-240) regarding the eleven steam generator tube plugs which were discovered to be missing during recent steam generator inspections.

The Company Nuclear Review Board and the Plant Nuclear Safety Committee have reviewed the incidents as described in our letter of July 27, 1977, (L-77-240) and have come to the following conclusions:

1. No loose objects were present in th'e reactor vessel immediately following the inspection.
2. The possibility exists that the presence of a single plug may have been missed during the inspection of the Reactor Coolant System for missing plugs, but the probability of eleven plugs being missed by the inspection is extremely small.
3. The .eleven missing plugs were most likely never originally installed.
4. A single object entered the reactor vessel following the inspections.

5. The object is metalic with a mass of between O.l 1.0 pound.
6. The object is now dormant in the reactor vessel, most likely at rest in a low flow area, within the bottom of the reactor vessel.
7. The operation of the unit with an object as described above in the reactor vessel has been thoroughly evaluated and it it has been concluded that will not, result in any adverse structural effects on the reactor vessel its cladding, or reactor internals. It has also been concluded that no adverse thermalhydraulic or other core conditions will result from the operation of the unit.

As a result of the foregoing conclusions, the Plant Nuclear Safety Committee and Company Nuclear Review Board have determined that the return of Unit 4 to full power operation 1) will not in-crease the probability or the consequences of any accident pre-viously analyzed in the FSAR, 2) does not create the possibility of an accident not analyzed in the FSAR, and, 3) does not decrease the margin of safety as described in the basis of the facility Technical Specifications. The PNSC and CNRB have therefore also concluded that the return of unit 4 to full power operation will not involve an unreviewed safety question and will not, adversely effect the health and safety of the public.

Moreover, to provide additional assurance that the unit is operating as expected upon return to power the following actions will be taken:

1) Low, intermediate, and high power physics" maps will be taken and submitted to the NRC within thirty days after reaching full power operations.
2) The use of the metal impact monitoring system (MIMS) with the capability,,of'detecting loose objects will be continued at least until the next reactor vessel inspection.

In the event that the MIMS is out of service, be reported to the NRC. Every effort will be made it will to restore the system to operation as soon as possible.

3) Any abnormal indications from the MIMS will be reported to'he NRC.

4) Within seven days of noting any abnormal indications, a written evaluation will be sent to the NRC.
5) Photographs of the steam generator tube sheets following the Spry.ng 1977 steam generator inspections will be sent to the NRC.
6) Following each startup from below 350'F core barrel movement will be evaluated using neutron noise techniques.
7) During the next major refueling, the reactor vessel will be inspected for loose parts and results reported to the NRC.
