I The Turkey Point Fitness for Duty Program uses a random testing rate of at least 50%.

Attachment 1 contains the Fitness for Duty Program performance data, as well as, a list of events reported and a summary of any management actions taken. Attachment 2 contains the Annual Fatigue Management Summary as required by 10 CFR Part 26.203(e).

Management Actions For the reporting period of 2011, twenty-one (21) individuals were denied unescorted access authorization after testing positive for drugs or alcohol. By test type, two (2) for-cause observed behavior tests were positive, seventeen (17) pre-access tests were positive, and two (2) random tests were positive.

For-Cause Observed Behavior Positive Test One (1) contractor employee tested positive for alcohol. One (1) licensee employee tested positive for alcohol. These individuals were denied access.

Pre-Access Positive Test Sixteen (16) contractor employees and one (1) licensee employee tested positive. One (1) initial contractor employee tested positive for alcohol. One (1) update contractor employee tested positive for alcohol. One (1) reinstatement contractor employee tested positive for alcohol. Two (2) initial contractor employees tested positive for cocaine. Seven (7) initial contractor employees tested positive for marijuana. Three (3) reinstatement contractor employees tested positive for marijuana.

One (1) initial contractor employee tested positive for both cocaine and marijuana. One (1) reinstatement licensee employee tested positive for marijuana. These individuals were denied access.

Additional Comments

1. There were no (0) subversion attempts during the 2011 reporting period.
2. For the 2011 reporting period, all blind specimens submitted to Turkey Point Nuclear Plant's HHS-certified laboratory yielded expected results. The third quarter of 2011, the blind specimen laboratory inadvertently sent two (2) adulterated specimens and no substituted specimen. The specimens received were submitted for a total of eleven (11) specimens with two adulterated and no substituted. This event was captured in the Correction Action Program.
3. One (1) individual was denied access for a violation of the conditional access/follow-up testing agreement requiring total abstinence from drugs and alcohol. The individual had an evidence of alcohol test.

Reportable Events Turkey Point Nuclear Plant had no (0) reportable events as defined in 10 CFR Part 26.719 during this reporting period.
