Dear Sir or Madam:

This letter submits the James A. FitzPatrick Nuclear Power Plant (JAF) Owner's Activity Report, Second Period, Fifth Interval which contains a summary of the lnservice Inspection (ISI) examinations conducted during the plant's 2022 Refuel Outage 25 (FP1R25).

Table 1 of the enclosed report summarizes the items with flaws or relevant indications that required evaluation for continued service and Table 2 provides an abstract of repair/replacement activities required for continued service.

This report is being submitted in accordance with JAFs approved Fifth Interval Inservice Inspection Program Plan [Reference 1].

CERTIFICATE OF INSERVICE INSPECTION I, the undersigned, holding a valid commission issued by the National Board of Boiler and Pressure Vessel Inspectors and the State or Province of New York and employed by The Hartford Steam Boiler lnsoection and Insurance Company of Hartford. CT have inspected the items described in this Owner's Activity Report and state that to the best of my knowledge and belief, the Owner has performed all activities represented by this report in accordance with the requirements of Section XI.

IR 04525962 F-A, F1.10c H10-344 Spring Can Spring can support on Residual Heat Removal found out of tolerance, evaluation determined support could perform intended design function in as found condition.

IR 04526268 D-8, D2.10 15-8"-VllES-151-100 [2022] A through-wall leak was identified on Class 3 Emergency Service Water piping.

ASME Section XI Code Case N-513 was employed.

IR 04524479 D-8, D2.10 15-3"-VllES-151-136 [2021] Exam showed a measured wall thickness below acceptance criteria. This wall thickness was evaluated for structural integrity until repairs could be made. See 21-005 in Table 2.

IR 04419896 D-8, D2.10 15-6"-VllES-151-103 [2021] A through-wall leak was identified on Class 3 Emergency Service Water piping.

ASME Section XI Code Case N-513 was employed.
