Violation:

' During an NRC inspection conducted on December 15,1996 through February 1,1997, a
violation of NRC requirements was identified. - In accordance with the " General
Statement of Policy and Procedure for NRC Enforcement Actions," NUREG-1600, the i
'
violation is listed below:
Criterion V of Appendix B to 10 CFR Part 50 requires, in part, that activities affecting quality be prescribed by documented instructions of a type appropriate
- i to the circumstances and be accomplished in accordance with these instructions.
i i
A sign, which provided instructions of a type appropriate to the circumstances,
!
installed on the entrance to the standby service water (SSW) pump room stated
that the SSW pumps were not protected by suction strainers and that all gaps in i
the floor must be covered during work to preclude dropping items into the SSW basin.
)
Contrary to the above, on January 17,1997, activities affecting quality were not accomplished in accordance with instructions in that all gaps in the floor were not covered during work to preclude dropping items into the SSW basin.
Reasons for the Violation:
The cause of this violation was that corrective actions from a 1991 condition report were
not effective in providing proper controls to assure that the floor penetrations in the standby cooling tower (SBCT) pump room were protected from the introduction of foreign materials.
- During the investigation into the root cause of this event, corrective action process changes, foreign material exclusion (FME) controls, FME culture, and operator awareness were evaluated. The corrective action culture has changed significantly since 1991. The Corrective Action Process was revised in 1994 and has resulted in a lower threshold for problem identification, more effective root cause evaluation and corrective action identification.
FME culture was evaluated through the use of a short questionnaire about FME. This survey was performed on a random selection of contractor and EOl craft personnel. The questionnaire provided a 100% positive response indicating a good awareness of FME controls and the actions to take if they should drop something into an open plant system.
Even though the operator did not see the sign posted on the pump room door, he showed i
proper concern and attention to detail for FME controls through his actions during the time that he was performing test activities in the pump room.

1. As an interim corrective action, the sign installed on the SBCT pump room door in 1991 was replaced with a larger (18x12 inches versus 10x8 inches), concisely worded (19 - 1" high words versus 50 - 5/16' words) sign until remaining actions are completed.
2. An Engineering Request (ER) was initiated to evaluate the appropriate method for

covering floor penetration openings in the Division 1 and 2 pump rooms of the SBCT.
Corrective Actions That Will Be' aken to Avoid Further Violations:

1. Install covering / barrier for floor penetration openings as determined in the ER.

1. Reinforce the importance of sign awareness and compliance with plant personnel.
Date When Full Compliance Will Be Achieved:
River Bend is currently in compliance, however long-term corrective actions will be completed by August 29,1997.
