Engineering Support Section 2 Inspection Summary Inspection on May 1-3, 1979 (Report Nos. 50-454/79-07 and 50-455/79-07 Areas Inspected: Pittsburgh Testing Laboratory nondestructive examin-ation (NDE) procedures, work activities, personnel certifications, and radiographs of field and shop weids.
This

Review of Pittsburgh Testing Laboratory NDE Activities a.
CECO hired PTL to perform all the NDE on field piping welds installed Hunter Corporation and Southwest Fabricating and Kelding Company, Inc.
(SFWC) and to perform all radiography on shop welds in accordance with ASME Section III, 1974 Edition with Summer 1975 Addenda.
b.
CECO and the Authorized Inspector (AI) review 100% of the radiographs and reports of field welds and Sargent and Lundy (S6L) reviews 100% and CECO reviews 10% of the radiographs and reports of shop welds.
c.
The inspector reviewed audits performed by a PTL auditor and CECO Quality Assurance (QA) en PTL activities.
d.
The inspector aleo reviewed PTL QA Manual QA-M-1, Rev. 3, 4/6/77, the prs _ dure for qualification and certification of PTL NDE technicians No. QC-PQ-1-NDE, Rev. 3, 9/8/77 and the following PTL personnel certifications.

Preservice inspection may start the Summer of 1980.

The inspec*or made a tour of the PTL f acilities escorted by the CECO QA L.ructural Coordinator and the PTL Site Manager.
The following are the inspector's findings.
(1) Radiographv (RT)
- The NRC License expires August 31, 1979, and covers IR 192 up to 150 curies and CO 60 up to 100 curies.
- PTL does not have a CO 60 source or a x-ray machine onsite.
- The Lab has four IR 192 sources stored in a locked concrete block room.
- PTL hauls the sources from place to place onsite in a pick-up using correct radioactive signs.
- A hand film processing tank is used to process f'lm.
- Survey meters are calibrated every 90 days.
- Docket dosimeters are read and recorded every day when performing RT.
- Film badges are processed and recorded every month.
- Acceptable color rope and signs were available for securing areas when radiography is being performed.
- Decay curves of the IR 192 sources are posted in lab.
- The lab uses Kodak industrial radicgraphic film only, Types I and II.
- No outdated film was observed.

- Film cassettes were in acceptable condition.
- Lead screens are checked every week and replaced if necessary.
- Fluorescent screens are not used nor are there any onsite.
- The film are stored in the dark room prior to exposure.
- The exposed film is stored in a brick, air conditioned, heated and humidity controlled vault.
- PTL uses lead numbers for the number belt and a flasher for the identification.
- The lab uses a X-2ite densitometer in acceptable condition.
- PTL uses a film density step wedge traceable to the Bureau of Standards for the calibration of the densitometer.
- ASTM penetrameters are the only penetrameters used onsite.
They were in acceptable condition and had certifications.
- PTL had various thicknesses of shims to be used under the penecrameters.
- The inspector reviewed Chicago Bridge and Iron containment liner plate welds 46-93 radiographed on 9/13/76.
- The inspector reviewed the following procedures and field and shop radiographs of welds in various systems.

Chilled Water in reviewing the radiographs of the shop welds, the inspector found that all the radiographs had the lead letter "F" placed on the penetrameters and not placed adjacent to the penetrameters as required by ASME Section V, 1974 Edition with 1975 Addenda, Article 2.
The licensee representative acknowledged the problem and placed a telephone call to SFWC.
SFWC informed CECO that approximately three years ago, a code inquiry was made to the American Society of Mechanical Engineers (ASME)
Committee on this subject and a reply was received stating that the lead letter "F" on the penetrameter was acceptable. Until a copy of this letter is reviewed by an NRC inspector, this is an unresolved item.

(2) Magnetic Particle (MT)
- The inspector observed magnetic particle examina-tion, yoke method, being performed on pipe welds, CC-20-1.
The inspector also observed the yoke, (Parker Probe Model 200) picking up a 10 lb. weight using alternating current -
a pole spacing of 3-6 inches.

ske was then turned to direct current and a 50 lb. weight was picked up with a pole spacing 3-6 inches.
This was performed in accordance with AFME Section V, 1974 Edition with Summer 1975 Addenda.

- The lab has two field prod units and four field yoke units in acceptable condition.
- PTL uses red and gray, finely divided, magnaflux magnetic particles.
- Procedure, Magnetic Particle Examination, QC-MT-1, Rev. 2, 9/8/77, was reviewed and found to be acceptable.
(3) Liquid Penetrant (PT)
- The inspector observed liquid penetrant examination, solvent removeable metbod, being performed on pipe welds FCI9.
- The lab usee Magnaflux materials and receives certifications.
- ?racedure, Liquid Penetrant Testing, QC-PT-1, Rev.
2, 9/8/77, was reviewed and found to be acceptable.
(4) Ultrasonic (UT)
- PTL has a Nortec 131-D ultrasonic instrocent onsite in acceptable condition including various calibration blocks.
- The lab uses Ceral couplant and has UT transducers in various s.izes, MHZ and angles.
- Ultrasonic Inspection Procedure No. QC-UT-1, Rev.
1, 9/8/77 and Ultrasonic Inspection Welded Structural Steel Procedure No. QC-UT-2, Rev. 1, 1/24/77 were reviewed and found to be acceptable.

Eddy Current (ET) Visual (VT), Leak Testing (LT)
To date PTL has not performed ET, VT or LT onsite.
No items of noncompliance or deviations were identified.
Unresolved Items Unresolved items are matters about which more information is required in order to ascertain whether they are acceptable items, items of noncompliance o-deviations.
An unresolved item disclosed during the inspection is discussed in Paragraph 1.f. (1).
