COMANCHE PEAK NUCLEAR POWER PLANT - NRC WRITTEN RETAKE EXAMINATION REPORT 05000445/2020301 AND 05000446/2020301

Dear Mr. Peters:

On June 10, 2020, the U.S. Nuclear Regulatory Commission (NRC) completed an initial operator license written retake examination at your Comanche Peak Nuclear Power Plant, Units 1 and 2. The enclosed report documents the examination results and licensing decisions.

The preliminary examination results were discussed on May 21, 2020, with Mr. J. Ruby, Examination and Simulator Supervisor. A telephonic exit meeting was conducted on June 10, 2020, with Mr. A. Marzloff, Director, Training, who was provided the NRC licensing decisions.

The examination included the evaluation of one applicant for a reactor operator license, one applicant for an instant senior reactor operator license, and two applicants for upgrade senior reactor operator licenses. The license examiner determined that all applicants satisfied the requirements of 10 CFR Part 55, and the appropriate licenses have been issued. There were no post-examination comments submitted by your staff. The enclosure contains details of this report.

No findings were identified during this examination.

The NRC examiner evaluated the competency of one applicant for a reactor operator license, one applicant for an instant senior reactor operator license, and two applicants for upgrade senior reactor operator licenses at Comanche Peak Nuclear Power Plant, Units 1 and 2.

The licensee developed the examination using NUREG-1021, "Operator Licensing Examination Standards for Power Reactors," Revision 11. The written retake examination was administered by the licensee on May 21, 2020.

The NRC examiner determined that all applicants satisfied the requirements of 10 CFR Part 55, and the appropriate licenses have been issued.

- INITIAL LICENSE EXAM

.1 License Applications

a. Scope

The NRC examiner reviewed all license applications submitted to ensure each applicant satisfied relevant license eligibility requirements.

b. Findings

No findings were identified.

.2 Examination Development

a. Scope

The NRC examiner provided the written retake examination outlines and reviewed the draft written retake examinations submitted by the licensee against the requirements of NUREG-1021.

b. Findings

The NRC examiner provided draft examination and post-validation comments to the licensee. The licensee satisfactorily completed comment resolution prior to examination administration.

The NRC examiner determined the written retake examination initially submitted by the licensee was within the range of acceptability expected for a proposed examination.

.3 Operator Knowledge and Performance

a. Scope

On May 21, 2020, the licensee proctored the administration of the written retake examinations to all applicants. The licensee staff graded the written retake examinations, analyzed the results, and presented their analysis to the NRC on June 4, 2020.

b. Findings

No findings were identified.

All applicants passed the written retake examination. The final written retake examinations and post-examination analysis may be accessed in the ADAMS system under the accession numbers noted in the attachment. There were no post-examination comments as indicated in the licensee submittal.

Post-examination analysis revealed seven generic weaknesses associated with applicant performance on the written retake examination:

  • Ability to predict changes in pressurizer spray valve differential temperature
  • Ability to determine rod position indication on a failure of the plant computer
  • Ability to determine pressurizer pressure control on loss of a vital AC instrument bus
  • Ability to predict a high flux reactor trip signal in response to loss of a source range instrument
  • Knowledge of use of the locked component deviation log
  • Ability to predict the location of a steam leak based on annunciator response
  • Knowledge of rod control system malfunction procedural requirement These deficiencies were captured in the licensees corrective action program as IR-2020-004038, IR-2020-004039, and IR-2020-0041. Copies of all individual examination reports were sent to the facility training manager for evaluation and determination of appropriate remedial training.

.4 Examination Security

a. Scope

The NRC examiner reviewed examination security for examination development and administration for compliance with 10 CFR 55.49 and NUREG-1021. Plans for examination security and applicant control were reviewed and discussed with licensee personnel.

b. Findings

No findings were identified.

Exit Meeting Summary

The chief examiner discussed the preliminary examination results with Mr. J. Ruby, Examination and Simulator Supervisor, on May 21, 2020. A telephonic exit was conducted on June 10, 2020, between Mr. C. Osterholtz, chief examiner, and Mr. A. Marzloff, Director, Training. The licensee did not identify any information or materials used during the examination as proprietary.
