Inspection on April 3-4, 1979 (Report Nos. 50-443/79-05 and 50-444/79-05)

Areas Inspected:

Routine, unannounced inspection by two regional based inspectors of construction activities. The inspection involved the main-tenance of material stored offsite.

A construction site tour was per-formed.

The inspection involved 20 inspector-hours by two regional based inspectors.

Results: Of the two areas inspected, one aprarent item of noncompliance was identified in one area (Infraction - Failure to electrically connect stored equipment supplied with space heaters as required in procedures -

Paragraph 3).

PlantTour The inspector observed work activities in progress, completed work, and plant status in several areas of the construction site. Work items were examined for any obvious de~fects or noncompliance with regulatory requirements cr licensee comitments.

Particular note was taken of the presence of quality control inspectors, and evi-dence of quality control activities such as inspection records,

,

material identification, nonconforming material identification, and equipment calibration tags. The inspector interviewed craft personnel and quality inspec' ion personnel as available.

Specific t

activities inspected included stripping forms in Unit 1 equipment vault, dewatering Unit 1 containment for liner plate installation, weld preparation on Unit 1 containment liner, and concrete compression strength testing in PTL building.

No items of noncompliance were identified.

3.

Maintenance of Stored Equipment at Newington, New Hampshire The offsite storage facilities, and stored equipment, at Newington, New Hampshire were examined during the inspection.

The conditions of storage such as environmental control, dunnage, rodent control, stacking, and segregation of nonconforming items were observed.

Electr: cal heaters are provided in the circuitry of certain motors.

These heaters are required by site directives to be energized at all times during storage. Monthly checks are required to insure that these heaters are energized. This is accomplished at the Newington warehouses by feeling the motor housings to detect tem-perature above ambient.

The inspector found no heater in operation on one of the 12 service water cooling pump motors. This lack of heat was confirmed by the licensee.

The inspector noted that the motor heaters were plugged into wall receptacles.

Lights in the receptacle supply circuits indicated only that the receptacles were energized, not that current was flowing in the motor heaters. This was demonstrated in the case of the service water cooling pump motor (mentioned in the preceeding paragraph) where the heater lead was plugged into the receptacle, the light was on, but the motor was cold. This circuitry is contrary to UE&C Procedure FGCP-9, Revision 4, March 9, 1979, Paragraph 6.1.5 which states, in part:

"Each piece of equipment equipped with a space heater will have service wiring connected to heater leads by means of a terminal block, wire nuts or other approved method."

Failure to implement documented instructions and procedures is an item of noncompliance relating to Criterion V of Appendix B of 10 CFR 50. This item is an infraction (443/79-05-01 and 444/79-05-01).

A 4-inch Grinnel diaphragm valve, Serial No. 75-1525-15-1, was observed in storage without the desiccant which is stated as a requirement in Westinghouse Procedure "WSS Component Receiving and Storage Guideline," Page 8-5, " Diaphragm Valve Receiving and Storage Instructions," March 1976.

However, UE&C Preventative Maintenance Book 5, Unit 2, No. 214-1, states that " desiccants are not applicable on 4-inch diaphragm valves". -

This item is unresolved pending.the licensee's resolution of the conflicting directives (443/79-05-02 and 444/79-05-02).

Salt Water Intrusion Durina Shipment of Equipment During the onsite receiving inspection of the reactor pressure vessel closure head and internals, evidence of small quantities of water were found by the licensee to be present under the coverings.

The liquid was analyzed for chlorides, and the ana-lyses confinned the presence of chlorides ranging from 11C up to 12,400 parts per million. Westinghouse approved the interim cleaning procedures to be followed.

At the time of the i tspection the cleaning operations had not been completed.

Nanconformance Reports (NCR's) Nos. 239, 240, and 241, were pre-pared on March 26, 1979; each of which included the notation

"Not reportable per 10 CFR 50.55(e)."

This item is unresolved pending review by an NRC inspector of the documentation of the inspec:; ion and cleaning of these components (443/79-05-03 and 444/79-05-03).
