Dear Mr. Ebneter:

Millstone Nuclear Power Station, Unit No. 3 Response to I&E Inspection No. 50-423/86-09 Pursuant to the provisions of 10CFR2.201, this report is submitted in reply to Reference (1), Appendix A which informed Northeast Nuclear Energy Company (NNECO) of an apparent Severity Level V Violation. This was the result of items of noncompliance noted during an inspection conducted from February 18, through March 14, 1986 by your office at the Millstone Unit No. 3 site.

Violation 10CFR50, Appendix B, Criterion II, " Quality Assurance Program", states in part:

"The program shall provide for indoctrination and training of personnel performing activities affecting quality as necessary to assure that suitable proficiency is achieved and maintained."

Administrative Control Procedure AC P-Q A-9.07, " Quality Assurance Surveillance Program", states in part:

" Inspector - An individual assigned to the NNECO QA/QC Department who is certified in installation inspection and test in accordance with reference 3.4 (ACP-QA-8.16 " Training, Certification and Identification of Qualified Inspection and Testing Personnel")."

" Prior to the performance of a surveillance the inspector should review the applicable documents, i.e.,... test... procedures."

ACP-QA-3.16, " Training, Certification and Identification of Qualification j and Testing Personnel", paragraph 6.1.3.1 states in part:

l

} " Training includes: (b) The procedures, specifications, codes and standards e i used during... testing activities...(c) The Quality Assurance elements to be i employed with guidance regarding their limitations and capabilities as specified in departmental, Quality Assurance or other training programs."

al - Contrary to the above on February 18, 1986, during the performance of start-up test, 3-INT-8000, Appendix 8023, Reactor Trip / Shutdown Outside j the Control Room, it was found that a QC inspector sent to witness this '

J test had not been trained or briefed on the applicable system or test

procedure and was not aware of the test's purpose or what was taking place i during the test. .

This is a Level V violation (Supplement I) applicable to NPF-49.

j The NNECO QA/QC Department has investigated this incident and has j made the following determination. The NNECO Surveillance Program 1 i utilized during the startup testing program on Millstone Unit No. 3 was a

!

very useful and effective management tool and the inspection personnel, were adequately briefed prior to performing surveillances. This was determined via discussions with Millstone Unit No.3 management personnel and the fact that many of the surveillances resulted in changes

,

j to work procedures and practices. However, in this case the inspector was i not properly briefed as to the scope of the surveillance he was to perform.

i it should be noted that in general the inspector may not be trained in every aspect of the evolution he is sent to observe. He should, however, be

,

briefed on the specific scope of his surveillance. For' example, during j

complex evolutions such as reactor physics testing, the inspector may only

be directed to verify compliance to the procedure for the portion of the l evolution he observes. The plant testing organization is responsible for the j technical adequacy of the procedure.
Cause

!

This is an isolated incident caused by the following factors. The testing evolution was complex and the timing of the performance'of the test was

<

such that the inspector was sent to cover the test on a "last minute" basis.

Adequate guidance as to the scope of the surveillance was not provided to the inspector by supervision.

] Corrective Action _s QA/QC Department has evaluated the incident and in order to strengthen the surveillance program a QA/QC Department level procedure is being

! developed. This procedure will provide additional direction to the j inspector supervising personnel on the preparation for and performance of

surveillances. This procedure will be implemented by July 1,1986.
