Site / Plant Tour:
The inspector conducted a walk-through tour of the site and the plant to assess general ccnformance to work procedures, house-keeping in open storage areas, and good construction practices in the areas of concrete, structural steel and material storage. The inspector also inspected stored materials in laydown areas for QC status tags denoting the accept / reject status of materials stored in open areas on the site, and the work areas inside the plant. The inspector observed that materials were stored properly in lay-down areas on supports and dunnege, pipe ends were covered / capped to prevent contamination, items were properly tagged indicating the accept / reject status, and the construction work in concrete and structural steel appeared to conform to project procedures and good construction practice.

Status of Construction Deficiency in Rock-Anchor System in Foundations of Category I Structures (50-423/79-00-01)
The inspector reviewed documentation, held discussions with cognizant constructor personnel, and visually inspected a random selection of accessible rock-anchors which have been installed to correct the deficiency. The inspector also verified the installation of additional columns in the auxiliary building to increase the structural stiffness of the building to resist seismic forces. Sestak, Jr., dated 11/7/79 Based on the review of documents, discussions with cognizant licensee and A-E engineers, and personal observation, the inspector determined that the licensee had performed extensive analyses of the construction deficiency reported to NRC, and had submitted the final report to NRC detailing the cause of the problem and the corrective actions required to alleviate the inadequacy of structural design.
It appears, however, that the statement in the final report of 4/21/81 is in conflict with the status of work on-site. The final report states on page 3 that,
"All works on the installation of columns and rock anchors have been completed" However, anchor nos. 18, 19 and 20 in the electrical tunnel were not complete as of October 23, 1981.
i Furthermore, NRC needs additional design information to establish the validity of re-analysis and the acceptaoility of corrective actions accomplished. This information was not available on-site, therefore, this CDR remains open and will be followed-up in a subsequent inspection.
