NOTICE OF VIOLATION AND ICE OF DEVIATION (Inspecti on Nos.
50-38 85-1 50-388/85-15)
This refers to the special NRC safety inspection conducted on April 22

May 8, 1985, at the Susquehanna Steam Electric Station, Salem Township, Pennsylvania which was in response to a violation of a Technical Specification Limiting Condition for Operation identified by your staff on April 21, 1985.
That violation involved the inoperability of one loop of the Emergency Service Water (ESW) System.
In addition, another violation identified by your staff relative to surveillance requirements on the FSW system was also reviewed duringlthe inspection, and a deviation associated with the safety related motor operated valve thermal overload discussion in the Final Safety Analysis Report (FSAR)
was identified during the same NRC inspection.
The inspection report di,scussing these items was sent to you on May 17, 1985.
On May 31, 1985, an enforcement conference was held with you and members of your staff to discuss the violations, their causes, and your corrective actions.
As previously stated, Violation A described in the enclosed Notice involved the inoperability of one of two ESW loops that service Units 1 and 2.
This occurred because a sliding link that disables the DC control power supply logic circuit for automatically actuating a bypass valve was opened for maintenance re'asons and not closed upon the completion of the task.
Because the control logic to the valve was lost, the valve would not open automatically as required on an ESW pump start.
This condition went unnoticed for approximately 17 days'hich also resulted in the Limiting Condition for Operation ( LCO) being exceeded.
Although the significance of this violation is low because (1) the valve~',was capable of being manually opened from the control room and (2) the other" loop was operable, the violation is of concern to the NRC because it indicates that there was not adequate sensitivity and administrative control over modification activities.

& Light Company Violation B refers to the licensee-identified non-performance of required valve alignments, pump flow tests, and valve exercise tests for the ESW system within the allowable surveillance interval.
The above two violations have each been classified as Severity Level IV violations in accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions,"
CFR Part 2, Appendix C (1985).
Nonetheless, we emphasize that similar violations in the future may result in additi'onal enforcement actions You are required to respond to the enclosed Notice and should follow the instructions specified therein when preparing your response.
Your written reply to this letter, especially as it relates to actions to prevent recurrence, and the results of future inspections, will be considered in determining whether further enforcement action is appropriate.
In accordance with 10 CFR 2.790, a copy of this letter and its enclosure will be placed in the NRC Public Document Room.
The responses directed by this letter and the enclosed Notice are not subject to the clearance procedures of the Office of Management and Budget, otherwise required by the Paperwork Reduction Act of 1980, PL 96-511.
