WATERFORD STEAM ELECTRIC STATION, UNIT 3 - INITIAL RETAKE EXAMINATION REPORT 50-382/02-302

Dear Mr. Venable:

On August 6, 2002, the NRC completed an initial retake examination at the Waterford Steam Electric Station, Unit 3. The enclosed report presents the results of this examination.

The examination included the evaluation of one applicant for a reactor operator license. We determined that the applicant failed the retake examination and did not satisfy the requirements of 10 CFR Part 55.

In accordance with 10 CFR 2.790 of the NRCs "Rules of Practice," a copy of this letter and your response will be made available electronically for public inspection in the NRC Public Document Room or from the Publicly Available Records (PARS) component of NRCs document system (ADAMS). -2-SUMMARY OF FINDINGS IR 05000382/02-302, on 7/31-8/6/2002, Entergy Operations, Inc., Waterford Steam Electric Station, Unit 3, initial retake examination of an applicant for a reactor operator license.

An NRC chief examiner evaluated the competency of one applicant for a reactor operator license at the Waterford Steam Electric Station, Unit 3. The applicant was administered a written retake examination that the facility developed using the guidance of NUREG-1021,

"Operator Licensing Examination Standards for Power Reactors," Revision 8, Supplement 1.

The written retake examination was administered to the applicant on July 31, 2002, by facility proctors in accordance with instructions provided by the chief examiner.

Report Details 4. OTHER ACTIVITIES 4OA4 Initial License Examinations

.1 Operator Knowledge and Performance a. Examination Scope On July 31, 2002, the licensee proctored the administration of a written retake examination to one applicant. The licensee staff graded the written examination, analyzed the results, and presented their analysis to the NRC on August 6, 2002.

The NRC granted a waiver to the applicant for the operating portion of the examination since the applicant had previously passed this portion during the initial operator license examination administered on January 21-24, 2002.

b. Findings The applicant scored 79.8 per cent and failed the retake written examination. The text of the examination questions may be accessed in the ADAMS system using the accession number as noted in the attachment.

The licensee conducted a performance analysis for the written examination with emphasis on three questions. The licensee concluded that there were no commonalities in the knowledge deficiencies. The licensee did not formally recommend any questions be modified or deleted from the written examination. However, the licensee stated that three questions would be modified in their examination bank. The chief examiner reviewed the technical basis for these questions as well as other questions missed by the applicant and regraded the examination. Question No. 39 was deleted from the examination since insufficient information was provided for the applicant to correctly answer the question. Questions Nos. 43 and 45 were revised to accept an additional answer choice, answer A. The chief examiner also reviewed all the other missed questions by the applicant and determined that the other questions were technically accurate and satisfactory as written.

No findings of significance were identified.

.2 Initial Licensing Examination Development The licensee developed the written retake examination in accordance with NUREG-1021, Revision 8, Supplement 1, using facility training and operations staff on the security agreement.

.2.1 Examination Outline and Examination Package a. Examination Scope The facility licensee submitted the written retake examination outlines on April 8, 2002.

The chief examiner reviewed the submittal against the requirements of NUREG-1021, Revision 8, Supplement 1, and provided comments to the licensee. The licensee revised the examination outlines and the chief examiner approved the initial examination outline and advised the licensee to proceed with the written retake examination development. The facility licensee submitted the draft written retake examination package on July 2, 2002. The chief examiner reviewed the draft submittal against the requirements of NUREG-1021, Revision 8, Supplement 1, and provided comments to the licensee on July 11, 2002. The licensee satisfactorily completed comment resolution on July 15, 2002. The licensee submitted letter W3F1-2002-0075 on August 6, 2002, which contained the examination analysis. The licensee letter may be accessed in the ADAMS system using the accession number, as noted in the attachment.

b. Findings The chief examiner determined that the written retake examination initially submitted by the licensee was within the range of acceptability expected for a proposed examination.

No findings of significance were identified.

40A5 Management Meetings

.1 Exit Meeting Summary The chief examiner presented the examination results to Mr. R. Fletcher, Operations Training Supervisor during a telephone conversation on August 12, 2002. The licensee acknowledged the findings presented.

The licensee did not identify as proprietary any information or materials examined during the examination.
