Millstone Unit 2 Licensed Operator Requalification Training Implementing Procedure, NTM-3.054, Revision 2 2.2. Program Review The inspector reviewed the-requalification program as described in NTM-3.054 and compared at to the standards for requalification programs designated in 10 CFR Part 55.

This review resulted in the determination by.the inspector that the requalification program is being conducted within the required 2 year cycle, In addition, the lecture topics and "on-the-job training" subjects

. referred to in 10 CFR 55.59 were verified to be included in the requalification program.

The inspector also reviewed attendance records for the 1987 requalification trainirg, cycles 1, 2 and 3 to determine if requalification students are attending scheduled classes and conpleting the required training. No deficiencies were noted.

2.3 : Observation of Requalification Training The inspector _ attended and observed two raqualification training

sessions; the Plant Computer Uninterruptible Power Supply' classroom training,'and the Steam Generator Tube Rupture Simulator E0P training.

During this review the following observations were noted:

The use of visual aids in the classroom session was impressive.

  • In conclusion, the training sessions attended by the inspector appeared to be both professional and effective. No deficiencies were noted.

2.4 Interviews of Licensed Operators The inspector conducted interviews with 5 operators, both R0 and SR0 license holders.

These interviews were aimed at determining the (

effectiveness of the program from the operator's viewpoint.

these interviews, the operators are generally comfortable with the

requalification program, and believe that their ~ training is sufficient to enable them to carry out their licensed responsibilities.

In general, operators see the need for continued attention in the area of procedural improvements. This concern has been recognized by management, who have designated a procedure writers group to rework various operations procedures.

This action is recognized as an ongoing evolution that is aimed at resolving problems encountered by the operators during implementation of various procedures.

Overall, all of the operators interviewed believe the requalification program is effective.

2.5 Parallel grading of Written Exams The inspector performed parallel grading on a sampling of five written exams for operators in the requalification program.

Results of the inspector's revie~w did not differ more than approximately 1%

of the licensee's grading. One exam reviewed by the inspector was graded correctly but improperly totaled, causing the final grade to be higher than the actual grade by several percent.

This error did not effect the pass / fail determination of the individual and is considered to be an isolated error.

Training management indicated that in the future, a second person will also perform the final grade determination so that an oversight of this type does not reoccur.

The inspector determined that written exam grading is performed satisfactorily and that management attention to the grading deficiency identified above is sufficient to prevent future similar occurrences.

2.6 Review of Operating Test Documentation The inspector reviewed a sampling of approximately 5 Operating Test results for completeness and overall adequacy.

The results of this review indicated that the records of these operating exams are adequate although several minor administrative oversights were observed. These minor oversights were discussed with the Training Supervisor who indicated that further direction will be supplied to the examiners in order to resolve inconsistencies in the documentation of operating ?xams.

No deficiencies were noted.

2.7 Control of Changes to the Requalification Training Program Nuclear Training Manual Procedure 2.06 governs the methods that MS-2 utilizes for incorporating changes to the training program. The

"Document Review List" of NTM 2.06 contains an extensive listing of reports, changes, and instructions from the utility, INPO, Industry Standards, and the NRC, etc. All of the documents listed on the review list are reviewed by Training Management for applicability to the training program.

Fig. 7.2 of NTM 2.06 is used to document whether training is required, states the recommended action. and records the actual corrective action performed.

These changes or additions are approved by the Nuclear Training Supervisor.

In addition a database containing a matrix of individual Fig. 7.2's and the utility training response is being maintained which allows easy retrieval and verification of these actions.

The inspector reviewed the licensee's corrective response to LERs 88-11 and 88-07. Training initiatives for these LERs included a new simulator scenario to be used in the requalification program, and/or classroom discussion of the events, as well as distribution of the LERs for operations personnel review. The inspector found the actions incorporated into the requalification training program in response to these LERs to be acceptable.
