SUMMARY: Requalification written examinations and operating tests were administered to two senior reactor operators (SROs) and two reactor operators (ROs). These examinations were administered in accordance with Revision 7 to NUREG-1021. All four operators passed their examinations, as graded by the NRC. The Calvert Cliffs Training Department failed an RO for his performance during the simulator scenarios based on the operator's failure to perform a thorough verification of plant status during two scenarios. These same four operators comprised a single operating crew. This crew was evaluated a satisfactory by both the NRC and the Calvert Cliffs Training Department during the conduct of the simulator scenarios. The licensed operator requalification training program was not evaluated because an insufficient number of operators were examined.
All three portions of the requalification examination were found to be technically challenging. A big improvement in complexity and format of the Job Performance Measures (JPMs) was noted. Managements involvement with evaluating.the crew performance during the simulator portion of the examination was a programmatic strength.
Written and operating examinations were administered to two SRO Instant candidates and eight RO candidates. All ten candidates passed their examinations and were issued licenses.
All ten candidates appeared well prepared for their licensing examinations. No generic weaknesses were noted.
Plant housekeeping improvements were noted by the examiners. Painting and lighting improvements to the lower turbine building deck were especially noted.

Written 2/0 2/0 4/0 Simulator 1/1 2/0 3/1 Walk-through 2/0 2/0 4/0 Overall 1/1 2/0 3/1 Calvert Cliffs failed an RO that the NRC passed. The reason for the facility failure was that the RO failed several times to properly verify the correct status of plant systems or components. For example, in one case, the Volume Control Tank level transmitter failed high, which caused Ixtdown flow to be diverted to the waste processing system.
Subsequently, actual VCT low level decreased to the point a VCT level alarm came in. It took the RO 25 minutes to discover that letdown had diverted to the waste processing system. In another case, the RO told the CRS that he had charging flow into the RCS when, if fact, there was zero flow due to Charging Header pressure being less than RCS pressure.
The NRC did not fail this RO because he had performed all his critical steps correctly, and the steps he failed to correctly perform were not of great safety significance.

The simulator scenarios used for the examination were challenging and provided the e
evaluators with ample opportunities for assessing the operators' skill.
The facility's management and evaluation crew did a good job of responding to some e
unexpected problems that occurred during one of the simulator scenarios. In that scenario, the crew took some unanticipated actions early in the scenario, which caused the scenario to take an unplanned route. Facility management and evaluation crew quickly identified where the scenario would most likely head and determined the crew critical tasks that could be used for evaluating the crew.
An improvement with the quality of the questions asked on the written examination
was also noted. The questions appeared to be more challenging and required more indepth knowledge of the areas examined.
B.
Weakness:
During the administration of the JPMs, some minor procedural problems were
identified with the plant operational procedures. In one case, the procedure, AOP-3B, directed the operator to a wrong step in the procedure and in another case the procedure, AOP-7D-1, directed the operator to a larger procedure, OI-3, but did not provide further specifics as to where in OI-3 the operator should go. It turned out that when the proper location was eventually found, it merely directed the operator to a third procedure, OI-2D. At least one operator never did find the correct location in OI-3 and eventually went to OI-2D on his own. The Operations Superintendent initiated efforts to correct these problems after he was informed of them at the exit meeting on October 14, 1993.

2.2 Programmatic Strengths and Weaknesses:
A.
Strengths:
All ten candidates appeared well prepared to take their initial examinations. Grades e
on the written exam ranged from 83 to 96 percent. The ROs demonstrated good board knowledge and skills during the simulator scenarios while the SROs maintained good command and control. Communication skills exhibited during the scenarios were generally above average. Few weaknesses were identified with the candidates during their plant walk-through.
The plant continues to make big improvements in its plant cleanliness program over e
the last two years. Painting and lighting improvements to the lower turbine building deck were especially noted. The general state of cleanliness appeared to be commendable.
B.
Weaknesses:
The candidates displayed weakness with the following questions in the written e
examination. Note: An identified weakness is when five or more candidates miss the same examination question.
Q.008 (RO) RO candidates had difficulty identifying the individual responsible for maintaining the Equipment Availability Section for purposes of the shift turnover.
Q.033 (RO) RO candidates had difficulty identifying the effect to the main feedwater regulating system if the feedwater bypass override reset pushbutton is momentarily depressed while operating at 100% power.
Q.062 (RO)/ The candidates had trouble identifying the effect caused by a loss of Q.056 (SRO) control voltage to the Atmospheric Dump and Turbine Bypass System.
Q.080b tRO) RO candidates had difficulty identifying the vacuum setpoint at which the Unit 2 turbine bypass valves become disabled.
Q.095 (RO)/ The candidates had trouble identifying the correct plant response caused Q.093 (SRO) by the loss of 125 Vdc Bus 11.

3.0 l'XIT MEETINGS:
On October 13, 1993, an exit meeting was held with the people indicated below to discuss
)
the observations made during the administration of the requalification examination. These i
observations are summarized in paragraph 1.2 above. At that time, the Operations l
i Superintendent indicated that he would initiate steps to correct the problems identified with the operating procedures.
On October 29,1993, a second exit meeting was held to summarize any generic concerns and observations the NRC examiners noted during the initial examination week. These observations are also summarized in paragraph 2.2 above. In addition, the following items were noted:
The training department provided good support during both the preexam site visit and f
the exam week. This support included providing good comraents on the written examination, identifying and correcting potential problems with the simulator scenarios and JPMs, maintaining the simulator so that few problems were experienced during the exam week, and providing simulate,r scenario stand-ins to augment the operating team when necessary.
The NRC stated they viewed as positive the proposed upgrading of the simulator
computer. As understood by the examiners, this proposed upgrade modification would allow the training department to add more potential plant incidents to simulator scenarios. The examiners noted that the limitations of the current computer had reduced exam flexibility regarding the range of malfunctions available.

While conducting the simulator portion of the operating test, there was one simulator fidelity problem noted. During the administration of a steam generator tube rupture event, the channel X digital display for Pressurizer level started indicating 60 inches lower than channel Y. The operators responded as if this was a normal malfunction that had been programmed into the scenario. The facility initiated a simulator work repair order after the scenario was completed.
The licensee is planning to upgrade the simulator computer in the near future. This upgrade will allow the training staff to simulate a larger number of potential plant incidents that should be useful for operator training and engineering purposes.

ATTACHMENT 3 Simulation Facility Report Facility Licensee: Calvert Cliffs. Units 1 and 2 Facility Docket Nos:
50-317 and 50-318 Operating Tests Administered from: October 25-29.1993 This form is used only to report simulator observations. These observations do not constitute audit or inspection findings and are not, without further verification and review, indicative of noncompliance with 10 CFR 55.45(b). These observations do not affect NRC certification or approval of the simulation facility other than to provide information that may be used in future evaluations. No licensee action is required in response to these observations.
While conducting the simulator portion of the operating test, there was one simulator fidelity problem noted. During the administration of a steam generator tube rupture event, the channel X digital display for Pressurizer level started indicating 60 inches lower than channel
"
Y. The operators responded as if this was a normal malfunction that had been programmed into the scenario. The facility initiated a simulator work repair order after the scenario was completed.
The licensee is planning to upgrade the simulator computer in the near future. This upgrade will allow the training staff to simulate a larger number of potential plant incidents that should be useful for operator training and engineering purposes.
