(RC INSPECTION REPORT NO. 50-289/90-34 Gentlemen.

This reft.rs to the inspection conducted by Messrs. W. R. Bennett, G. A. Pick, and R. P. Mullikin of this office during the period October 17 through

-

November 26, 1990. The inspection included a revlew of activities authorized

'

for the Cooper Nuclear Station facility.

At the conclusion of the inspection, the findings were discussed with Mr. J. M. Meacham and other members of the-plant staff identified in the enclosed inspection report.

Areas examined during the inspection are identified in the report. Within-these areas, the inspection consisted of selective examination of procedures and representative records, interviews with personnel, and observations of activities in progress.

Based on the results of this inspection, certain of your. activities appeared to be in violation of NRC requiremnts. The violation involved the failure to use an adequate procedure for perfonnance testing. However, in accordance with SectionV.AoftheNRC'sEnforcementPolicy(10CFRPart2.AppendixC),no

citation is being issued for the viciation described:in paragraph 4.a of the enclosed inspection report. Your prompt and thorough response toithis issue a was a key element in our decision. Although this was an isolated incident and programmatic controls should prevent recurrence, we are concerned that a-procedure was not changed to reflect plant coaditions after installation of a.

To address this. issu:, Mr. J. M. Meachan committed to review all' outstanding design changes to ensure that procedure changes are being implemented and to

issue.a letter to the plant staff reminding them of their responsibility to-

,

ensure that procedures are adequate.

,

In addition, Mr. Meacham comittcd to perform an evaluation of Ibng-term,

. human-factors corrective actions concerning cross-connecting reactor equipment

~. cooling and turbine equipment cooling systems prior to the next scheduled refueling outage. The details of 'ths consnitments are discussed in-paragraph 3.a of the enclosed report.
