This refers to the special safety inspection conducted by NRC inspectors Mark M. Holzmer, Norman R. Williamsen, and Patricia L'. Eng of this office on May 8, 1987 through May 12, 1987, of activities at Zion Nuclear Power Station, Units 1 and 2, authorized by NRC Operating Licenses No. DPR-39 and No. DPR-48 and to the discussion of our_ findings with you and members of your staff at an enforcement conference at the Zion Station on June 19, 1987.
This inspection concerned the April 30, 1987, Unit I safety injection caused by the unplanned opening of all four main steam isolation valves (MSIV).
The valves opened when the MSIV hydraulic dump valve fuses were removed out of sequence. During the enforcement conference you stated that the cause of the event was poor communications. We agree that poor communications practices, including failure of operations personnel to repeat-back orders, were a primary contributor to the event.
In addition, we believe that failure to follow procedures for operating the main steam system and for

out-of-service ta0ging also contributed to the event.
We classified this event as an Unresolved Item in Inspection Report No.
295/87012 forwarded by our letter dated May 27, 1987.
(This report number was later changed to No. 295/87013 due to a clerical error.which assigned this report number to two different inspection reports.)
In our letter, we stated that the NRC was reviewing this item in regard to possible future enforcement action. That review is now complete.
Based on our review of this matter, we have concluded that the identified deficiencies were violations of NRC requirements as specified in the enclosed Notice. A written response is required.
In accordar:ce with 10 CFR 2.790 of the Commission's regulations, a copy of this letter, the enclosed Notice of Violation, and your response to this letter will be placed in the NRC Public Document Room.
